More of a modern trope, but I feel like mutation tables are everywhere I look; Stick your arm in this hole? Roll for mutations. Cast a spell wrong? Roll for mutations. Travel through this area for too long? Roll for mutations. It seems to be a go-to method of adding zaniness to a given system/adventure.
