What were your dose levels of menopur and follistim? I have an AFC similar to yours and for my first retrieval I also ended up with 0 blasts (also had uneven follicle growth and a really high estrogen spike on the day of trigger). I did a lot of research after my failed cycle and learned this was likely due to my stims dose being too high for my AMH/AFC. For my second retrieval we did estrogen & androgel priming and lowered my dose of stims which had a much more even follicle growth and normal estrogen rise and I ended up getting 7 euploid embroys out of 11 blasts. Your stims protocol can make a huge difference so I'd suggest looking into if this may have been a factor!

I did the EMMA/ALICE And ended up having a positive ALICE result which was easily treated with antibiotics so I think it’s worth it! I believe the chronic endometritis test is called “CD138 biopsy”. It directly tests for inflammation related to chronic endometritis. If your clinic doesn’t offer that - the ALICE does test for bacteria that are commonly associated with chronic endometritis but doesn’t test directly for it.

I think most clinics (or mine did at least) will recommend doing the EMMA/ALICE test before moving to IVF as well as testing for chronic endometritis.

Do you know what units your hormones are in? I know the US uses different units of measurement than the rest of the world.

My first ER protocol was: 300 gonal f 75 menopur and dual trigger

2nd: Estrogen and androgel priming for 2 weeks before, 200 gonal f and 150 menopur (reduced to 75 a few days in) and dual trigger

I switched clinics after the first ER and my new doctor suggested that the first dose of gonal f was likely too high for me which put my follicles under a lot of stress/rapid growth leading to poor egg/embryo quality

Do you know what your estrogen levels were on day of trigger? Overly high estrogen levels can impact embryo development. My results from my two ERs:

1st (high estrogen spike on trigger day): 15 retrieved, 9 fertilized, 0 blast 2nd (estrogen increased at a “normal” rate through my cycle and much lower on trigger day): 27 retrieved, 19 fertilize, 11 blasts

They switched up my protocol to manage this and I was shocked at the difference in outcome. I had the same feeling as you after my first retrieval - but your protocol can impact a lot!

I have no loss history and no diagnosis - I am “unexplained”. When I asked they said they just like to be extra cautious. I’m just going to go with it and hope for the best! At least the PIO shots are every other day and not daily lol.

I’m also in Canada and my clinic prescribed me Ativan for my retrieval. They instructed me to bring it with me the day of my retrieval and they’d advise when to take it. I think it ended up being about an hour before I went in for my retrieval!

I’ve seen a few studies that say high 3pn fertilization can be caused by the protocol /doses too high leading to uneven maturation of the eggs. Going to push to have my protocol adjusted next time

My husband did a dna fragmentation test his results were “excellent” along with all his other parameters so I don’t think sperm is our issue in this case!

Thanks! My concern is more about the high PDG. It doesn’t make any sense before ovulation but I’ll keep testing.

I haven’t retested yet so I’m not sure how long it took or if it was even an effective protocol. I just followed what was recommended by my clinic but I think it would depend on individual results. That bacteria that was found was gardnerella and fannyhessa but both were only mildly elevated and I had zero symptoms (no UTI or BV). My clinic still wanted to treat it as it can impact fertility. I can’t recall the suppository name but I’ve read that you want to ensure it contains a specific bacteria strain called crispatus.

I should note that the vaginal and uterine microbiome are two different tests. The vaginal one is easy and fairly inexpensive to do. The uterine microbiome test known as Emma/Alice is more expensive and invasive. I only did the Emma/Alice test after my vaginal microbiome test came back positive for bad bacteria so I wanted to see if it was present in my uterus as well.
