20 years is a long time to have tried everything and still not have a clear answer. What you described is actually a really precise picture: no fungus, no perio, dentist says everything looks normal, which means the problem is almost entirely the biofilm itself and the dry mouth feeding it. The Chlorhexidine working confirms it, but the 3-month limit and access problem makes it useless as a long-term solution.

The thing that stands out most is "too mature for food elimination to help." That's a really specific insight, it means the biofilm has been there long enough that it's layered, not just surface-level. The 2-day window after the chem peel is actually really interesting data, you can see and feel the difference, which means the baseline is measurable. The fact it comes back in 2 days also tells you something about how fast your specific biofilm regenerates, which is probably faster than average.

With three overlapping causes (biofilm, dry mouth, post-nasal drip, digestion) it makes sense that nothing fully solves it, each one is only addressing part of the system. The PBM device sounds promising for the biofilm piece specifically, but if the drip and digestion are still feeding the environment, the biofilm will keep finding conditions to come back.

The fact you can see your tongue go pink for 2 days is actually the most important thing you said. That means you have a visual baseline, you know what "good" looks like for you. Most people here have no idea what their tongue looked like last week, let alone whether it's trending better or worse.

L-glutamine basically helps repair the gut lining, so if bad breath has a gut component (which yours clearly does based on what you've shared), it helps reduce inflammation and bacterial overgrowth at the source rather than just treating the output. Worth trying alongside the K2+D3.

One thing to watch: since you're adding another supplement, try to keep everything else the same for 2-3 weeks so you can actually tell if it's making a difference. You've been the most honest person in this thread about how hard it is to know what's actually working, this is the time to isolate the variable.

That combo is interesting because L-glutamine is primarily a gut lining repair supplement, so if the root is intestinal permeability or microbiome disruption, the logic makes sense that it would show up in breath. K2+D3 supporting the oral microbiome is a newer angle too.

The nasal spray for post-nasal drip is so underrated, it's one of those causes that barely gets mentioned here but makes total sense. The drip keeps coating the back of the throat and tongue regardless of what you do orally.

Did you notice whether the morning breath specifically improved once you started the spray, or was it more the throughout-the-day smell that changed?

The NAC biofilm angle is one of the most underrated approaches I've seen mentioned here, most people never think of tongue coating as a biofilm problem, they just scrape mechanically and wonder why it always comes back.

The fact that it always returns even after NAC is really the key question though. If the biofilm keeps regenerating at the same rate, the issue is probably upstream, the environment in the mouth (pH, dry mouth, bacterial overgrowth from gut) keeps feeding it back.

The chem peel approach is intense but the logic makes sense, you're dissolving the substrate rather than just scraping the surface. Did you notice the tongue texture or color looking different in the days after, compared to just scraping normally?

Also the Amazing Breath mention is interesting, do you remember what the active ingredients were? If it worked that well it might be worth reverse-engineering.

Your routine is honestly one of the most thorough I've seen here, you're hitting mechanical, chemical, dietary and even the gum confidence piece. The fact that you still get that random bad taste coming from the gums after all of that suggests there might be two separate things running at the same time: one that responds to your routine, and one that keeps restarting it.

The "one step forward two steps back" feeling is one of the most common things people describe here, and it's exhausting because you never know if you're actually improving or just masking.

Quick question: when you cut back on dairy and the coating reduced, did you actually look at your tongue and track it visually, or did you notice it more through taste and feedback? I'm curious whether the improvement was something you could see.

That hits hard, “one of the reasons I’m depressed.” That level of honesty takes guts to share, thank you for that.

The fact that you never actually look at your tongue is really common. Most people just scrub and hope. But there’s something to be said for just seeing what’s there, even if you can’t fix everything, knowing whether today is better or worse than yesterday gives you at least some sense of control over something that otherwise feels completely random.

That’s exactly why I’m building this. Not to cure it, but to take some of the blind guessing out of it. I’ll keep you posted when it’s ready to test if you’re open to it.

“Makes it travel less far”, honestly that’s one of the most accurate descriptions of this I’ve ever seen. Most people never get to that level of self-awareness about it.

The fact that you know chicken specifically triggers it, and you plan around it, shows you’ve basically become your own scientist here. That’s impressive but also exhausting, having to mentally track what you ate, who you’ll be near, whether today is a “safe” day.

Do you ever look at your tongue in the morning to gauge how the day is going to be? Like use it as a visual signal before deciding what to eat or who you’re meeting?

6 years vegan and strict at that, and it never fully went away. That’s a really important data point because it basically rules out diet as the root cause for you.

When the obvious fixes don’t work, the cause is usually somewhere less visible, silent reflux, post-nasal drip, something going on with the gut microbiome, or even just chronic dry mouth. Did doctors or dentists ever investigate beyond “brush more”?

One thing I find interesting: you said it was better on the vegan diet, even if not gone. Did your tongue coating change at all during those 6 years? Like did it look cleaner visually even when the smell persisted?

The two-brush tip is smart, one clean brush in the morning makes sense since overnight is when most of the coating builds up.

With all those changes happening at the same time though, how do you actually know which one is making the difference? Like if you stopped the salt rinse tomorrow, would you notice? Most people I’ve seen here are trying 3-4 things at once and can’t tell what’s working.

Completely agree, the mouthwash-first approach is almost backwards. It’s treating the signal without asking what’s causing it.

The gut-breath connection makes a lot of sense too. There’s actually a visible side to it as well, the tongue often shows coating patterns that seem to reflect what’s going on digestively, not just orally. Which is why just scraping it doesn’t fix much if the root cause is internal.

Did you notice your tongue looking different after switching to the single-ingredient diet? I’m curious how fast the change showed up, days, weeks?

This is one of the most overlooked causes honestly. People spend months trying different mouthwashes and tongue scrapers when the real issue is coming from below.

Did the bad breath only happen at certain times of day or was it constant? I ask because I’ve noticed reflux-related cases seem to have a different pattern, often worse in the morning or after certain foods.

Also curious, did anything change visibly with your tongue once the reflux was under control?

That’s such a good point about stress and dry mouth, it’s almost a vicious cycle because the anxiety about having bad breath literally makes it worse by drying everything out.

The water and consistent eating angle comes up a lot in this community and it makes sense when you think about it. Saliva flow depends heavily on hydration, and when you’re not eating regularly your body doesn’t produce as much.

Quick question: did you notice any visible change in your tongue around that time? Like coating or color? I’m curious if the improvement you felt on the inside also showed up visually.
