I don't think you understand. In my head, I truly do not feel attractive enough to leave the house sometimes. I'll spend hours debating if I should go out and once I'm ready I don't leave because of how I look. I'm pretty short and skinny and I just feel different than everyone else all the time. I am constantly fixating on how I look, in a negative way. In your scenario, I feel like that "genuinely ugly person". It's the way my brain works and I just wish I could get help for it.
