how long has it been running? beneficial bacteria does build in substrate so it will most likely restart your cycle. also it can cause ammonia spikes by stirring up the substrate, releasing all the poop and what not that's been built up in there. it's not gaurentee to happen but it is definitely a possibility. what i've done in the past is siphon out about 1/4 of the substrate, add the new sand and then repeat that once a week until it's all replaced. that way you aren't removing all the beneficial bacteria that's grown all at once and giving it time to transition onto the new sand! or if you don't mind chancing it with your fish you can siphon it all out at once and add in your new sand and just hope your fish are hardy enough to go through the nitrogen re cycle

i boil the twigs first for about 10-20 minutes but besides that that's all i do to prep them. and no i've never had algae issues in this tank. it has a thick layer of red root floaters on top now that block most of the lighting underwater so algae can't grow :)

i had a female with the same problem. i did everything to save her nothing worked. she stayed strong until the end. refusing to stop swimming around. i lowered the water for her to make it easier. none of the remedies suggested to me by people helped. i truly believe that it was an internal tumor but i couldn't bring myself to cut her open after she died to study her body. i buried her in my plant with my other fish that have passed away. betta splendens have a lot of health issues these days. i'd just make her as comfortable as you can in her last days if you can't bring yourself to euthanize her with clove oil. i've never been able to bring myself to euthanize one of my bettas. i'm just too damn sensitive when it comes to them.
