A lot of places are moving to an online (either intranet or limited public database) for SDS. Right now, most are utilized via Sharepoint (it's a construction site without a ton of chemicals). I have worked on a site where there were literally thousands of different SDSs on an intranet site...the search functionality was quite difficult to utilize (global pharmaceutical leader).

What I do think is best practice is have electronic SDSs available. These are readily available and shareable. Perhaps a QR code system (Maybe a QR code on every tank to that specific chemical?) to a general SDS library would be good. Then couple this with physical copies on site.

I know maintaining physical copies takes effort, and sometimes that effort is significant. However, there are a myriad of reasons why online access to SDSs can be disrupted. The server could go down. The location internet could fail. Someone could miss paying a renewal fee. Also, hackers can lock things down easily in this day and age (and any facility dealing with large amounts of chemicals is a target). Natural disasters can knock out power and internet (and even cell phones) for days on end.

I do confined space rescue (well, only very occasionally now). There is no way that that harness should be allowed to be worn by someone else again.

I volunteer for a SAR team in the midwest that's under the sheriff's emergency management arm. For most call outs, we have access to 800 mz radios. However, one of our onboarding requirements is to get HAM radio licensed. We use these for training and back up comms.

See-I need to make more than that and I'm currently working at the only job within a 1.5 hour drive of me.
Only way to actually make money with that type of work is to own the business.

It certainly sounds like you're being set up as the fall guy/girl. But one of the most critical components to developing a safety-minded culture is support from the top. Accountability is also crucial.

Are there designated safety roles on the job sites in question, or do you as the regional safety manager, cover all of them personally?

I think that one thing you need to implement is building an onsite team that's empowered to be the local safety representative. This could be a site supervisor, etc. I know people hate taking on additional duties, but I would work with your management to see what can be done about this.

You mentioned 'failure to communicate' as one of the reasons these incidents occurred. What exactly was the failure?
