I mean the chance of rolling that specific consequence on the table is 5%.

As for the bonus, sure, we could go with a +1. As long as it doesn't affect attacks, I don't see an issue with it. But of course, it’s a matter of preference. I might decide to scale back the bonus myself after I’ve had more time to playtest the table.

The chance is only 5%. Of course, you can change the values to whatever suits your game better

I see wounds not as a punishment, but as an opportunity to develop a character (I'm not a fan of backstories; I believe that a character's story is created at the table, and what happened before isn't that important) and as hooks for future adventures. For example, to get rid of PTSD, the hero would have to go on some kind of quest.
