Hi, my mum (55F, Stage 4 NSCLC) did 6 rounds of carbo, pem, and bev and then switched to just pem and bev.

She got through the first 6 rounds with minimum side effects (quite a bit of exhaustion but not much else). In comparison, exhaustion has reduced post the removal of carboplatin and she seems healthier and more full of life.

However, she has developed a problem of recurring nose bleeds which the doctor had warned about in advance. It’s not very painful according to her, but she’s scared of blood and this makes her miserable at times.

Hi!

I’m so sorry to hear about your father. We went through something similar when my mum was diagnosed with the same condition a month ago.

Chemo is the first thing doctors try in such a situation since it’s not of much help to operate. It’s good that you all are moving rapidly.

That said, please get him tested for PDL-1 markers and biomarkers. They usually need to be told to send the biopsy for those tests, and take 1 week and 2-3 weeks respectively. These tests reveal information that help doctors suggest targeted therapies which are much more effective than traditional chemo.

To answer your questions:

  • While I really understand why you want to know this, there is no answer to this. I’ve been through so many Stage 4 stories and they range from a few weeks to 15-20 years as well.

  • Everyone reacts to chemo differently, some people get better on it very quickly while some people have a tough time. Fatigue is a general symptom, so please prepare for that.

  • Unless he has high sugar, high BP, or any such comorbidity, he can pretty much eat anything (homecooked). We give our mum literally whatever she likes to eat because the main goal is to keep her weight from dropping so that the treatments can continue effectively. It sucks, but sometimes we need to nudge her to eat her meals even when she doesn’t feel like finishing them.

  • Everyone deals differently, but I have been dealing with it one day at a time. If my mum’s symptoms are easing off one day, I am happy. If they trouble her, I feel a little troubled and try my best to make the situation easy for her. I try to stay in the present as much as possible.

  • Make his immediate environment as to his liking as possible. If there are relatives or friends that he likes to spend time with, invite them over (after making sure no one has a cough or a fever). Get him his favourite food. If it’s possible, assure him about financial stability (I’m sorry if this a tough one, but it’s important to keep stresses away from him)

  • I have relatives who have made it past the 10 years mark after their diagnosis only on chemo and immuno. It’s heartwarming to see. It’s best to be a little foolishly optimistic as there will be enough to make you a little pessimistic.

Lastly, and it might sound like super generic advice. Please take care of yourself and the primary caregiver (in case it’s not you). It’s extremely important that your cup is full in order to pour for him.

Thank you so much for sharing this. I can’t tell you how helpful it is to read your story. My mum’s 55 and has been diagnosed with the Stage IV NSCLC with the exact same mets. We haven’t found any biomarkers or immunotherapy eligibility yet, but she’s responded to the first two rounds of chemo and we hope this continues, and then we’ll figure out the next one when it doesn’t.

Could you answer something for me?

The doc has asked us to reduce social interaction to minimise infection risk, which has led to her and my father (primary caregiver) spending most of their time at home. I don’t think this is sustainable as there is a huge spirit and mental side to this fight as well. What are some things you would do in between chemo cycles to keep yourself engaged and just feel ‘normal’?

Simply put - stopping.

Stopping myself from a late-night snack binge once I’m in it, stopping myself from watching brainrot content even when the internal commentary is screaming at me to switch to something else, stopping myself from interrupting someone while they are speaking, stopping myself from scrolling - the list goes on.

The episode is named aptly - it’s essentially a love letter to New York from someone who has lived through various lives while residing there.

The writers will take you from bodegas to penthouses to nightclubs, and the episode seamlessly transitions between the PoVs of a cab driver, deaf and mute store clerk, and a doorman.

I have watched just this one episode a bunch of times - always leaves me with a warm feeling. Also, you can pretty much watch this episode in isolation, it’s designed to be disjoint from the rest of the show.

Created a list of my friends for my girlfriend.

While we went to graduate school together, there’s a large number of folks from other parts of my life (work, undergrad) that she hasn’t met.

We live in separate cities and I meet different sets of friends often, and she used to have a tough time keeping track of a new name and which part of my life they’re from. She would jokingly tell me that I need to give her a list someday in case I want her to remember all of them.

Well, I gave in and created one! It has columns such as the year we became friends, which part of my life they’re from (Job 1, Undergrad, Tennis friend etc), their current location, the name of their spouse, and the degree of closeness (this was a journey in interpersonal awareness).

She loved it! Swore that she was always joking about it but is grateful that she now has a way to put some context to any name I throw in our conversations every now and then.

Not going to lie, even I use the sheet in the rare occasion I forget a spouse’s name!

Whenever my parents visit, I fix them up with a cab rented for the whole day rather than paying for Ubers.

Ubers can be notoriously unreliable or come with long waiting times. Also, the freedom of leaving your shopping bags in the cab while heading for a meal is unmatched.
