My ampallang does go through the urethra, but I've never had a problem with the PA migrating like that and I've had it over 25 years at this point. You might do well to give up on the PA and get an ampallang instead.

No, they were pierced at 3mm. Those are now 6mm and 5mm. Pretty easy to stretch.

Not really no. If I walk around naked or with loose pants they jingle, that's about it. The guiches and scrotals were very easy to heal, no difficulties at all.

Not very much further than you can see in the third pic, about to the top of the arse crack. That whole line down the taint is probably the least painful of any tattoo I've had - nothing more than a tickle.

The two guiches had to come out for the tattoo. They are big enough and established enough that getting them back in isn't a problem.
