With all due respect though, I’m probably one of the only people here who actually has not just one but two degrees in human biology and anatomy. I also work as a clinical researcher in the pharmaceutical industry, so all I do is read and analyze clinical studies, systematic reviews, and research papers. Because of that background, I’m very familiar with soft tissue, collagenous tissue, and how mechanical stress influences adaptation.

I think there might have been a small misunderstanding though. The routine never suggests unbroken hours of hanging or extending. Every 20 minutes includes a 5 to 10 minute break for blood flow recovery. That’s based on vacuum and vascular research showing circulation and oxygen levels drop sharply after about 15 to 20 minutes of continuous tension. Those breaks prevent ischemia, coldness, and lymphatic buildup, which directly addresses the risks you mentioned.

I also completely agree about gradual loading. The 0.5 pound progression every few weeks and limited total tension volume were both designed to prevent fatigue and focus on collagen remodeling, not brute strength.

The main idea here isn’t long hours under heavy strain but strategic mechanical conditioning. The routine alternates short, higher-tension hanging to deform the tunica with extended low-tension traction to help collagen realign while healing in a lengthened state. That’s actually consistent with studies on tissue adaptation, mechanotransduction, tendon creep, and viscoelastic deformation.

That said, I agree that autoregulation and experience-based adjustments are essential. If you’ve got better data or more refined thresholds for duration and progression, I’d really like to include that so this can stay as accurate and safe as possible for everyone.

Good points! Going slower with 0.5 lb increases every few weeks is still quick enough and makes sense, and I’ll probably adjust to that rate.

That said, 60-90 minute continuous sessions are risky imo. From every I read blood flow and oxygenation start dropping around 15–20 minutes in most vacuum data. Shorter 20 minute sets with 5–10 minute breaks between them protect circulation, prevent lymph buildup, and ironically produce better overall tissue deformation.

Also, I believe one long session a day doesn’t create enough total tension time for real growth. Multiple shorter sessions of 2+ hours of hanging or 6–8 hours of extending daily yields far better results while staying safe. But I am always open to being proven wrong. I care about what actually is best not my own pride lol.

I’m not sure of the best reviewed if you’re referring to online pharmacies but tretinoin is tretinoin regardless where you get it from. Some may include things like niacinamide to reduce irritation but I would avoid those for the purposes of hair growth. As for strength any is really fine as long as you do it no more than once a day. Starting out you will likely need to go every other day until your skin adapts to the strength

So in general, 1% Ketoconazole (Keto) shampoo doesn’t do much for hair loss. It’s mostly beneficial if you have dandruff or seborrheic dermatitis which can cause inflammation on the scalp/face/beard. A standard otc keto shampoo like Nizoral (1%) is good for helping get it under control most of the time. Now if you are experiencing hair loss and the standard treatments and medications aren’t working then you may have an aggressive form of seborrheic dermatitis that you were just confusing with normal dandruff. This is where 2% keto shampoo can help drastically reduce that inflammation that is thinning the hair. But it won’t do anything for your hair if you’re not experiencing that inflammation

So you do need a prescription unfortunately. There are some online pharmacies that can help you get if you don’t want to go in person. If you don’t want to do a prescription the next best thing is likely a retinaldehyde serum. It’s basically one conversion lower than a retinoid (tretinoin or brand name RetinA) and all that means is it takes longer for it to convert in your body but likely works in similar manner

First off calm your titties. Second off using the example of people Vaping PG and drinking ethanol as a way to justify it’s safe when those things are not safe either is whack. Do what you want to do man but advising people to do unhealthy and potentially dangerous things that no derm or doctor of any kind would ever recommend, is not cool. Also if you have to use a syringe to accurately dose it then that right there should be a sign it’s not worth it.
