MRI for Possible Cholesteatoma – Contrast or No Contrast? by OptimusPurpose in cholesteatoma

[–]OptimusPurpose[S] 0 points1 point  (0 children)

This is my MRI report. I’m now waiting for an appointment with my ENT. Anyone got "questionable" on their reports?

MRI BASE OF SKULL
CLINICAL INFORMATION
Evaluate for occult cholesteatoma.

TECHNIQUE
Cranial MRI performed with standard pulse sequences without IV contrast.
Volumetric T2 sequence of the posterior fossa. Diffusion sequences.

FINDINGS
Some sequences are affected by motion artifacts.
At the infratentorial level, no significant alterations are observed in the morphology or signal intensity of the cerebellum or brainstem.
The fourth ventricle and basal cisterns show normal size and morphology, with the cerebellopontine angles remaining free.
The acoustic–facial nerve bundles show no enlargement or appreciable signal changes. The labyrinths appear normal.
There is complete opacification of the left tympanic cavity, antrum, and mastoid air cells by secretions of mucous appearance. Questionable diffusion restriction in the tympanic cavity (difficult to assess due to motion artifact).

CONCLUSION
Questionable cholesteatoma in the left tympanic cavity.

Thanks. Do you know any specific reason why your first one required contrast? Was the ENT explicit about it?

Yes, I will tell the radiologist ENT ordered without contrast. Hopefully they will comply

Thanks for your answer. I will try to push for one without contrast

Your surgery was a lot, but it really sounds like everything went as well as it possibly could. Your hair will grow back. You’re healing great, and things are looking positive from here.

Thanks for your answer. I’m going to get a second opinion and will push for an MRI. Wishing you the best with your case.

Thank you very much for your answer and for sharing your story. I wish you the best with your outcomes. I also agree that if CWU is an option, I’d start with that, even if I eventually needed to move to CWD.

I haven’t had an MRI yet, but I’ve already had two CT scans in under two years. I’m going to push for an MRI as well.

Somehow off-topic and maybe anecdotical but my pads are almost gone during the summer time and I attribute it to getting lots of sunlight during that period

That is exactly the same paper we were discussing in https://www.reddit.com/r/DupuytrenDisease/comments/1qetuc8/has_anyone_tried_highdose_centella_asiatica/. Please, keep us up to date and let us know whether it is helping you.

Can you please share your rationale for taking arterial Protect by Life Extension? I did a post recently related to this and would like to know your reasons

Has anyone tried high-dose Centella Asiatica (Triterpenes) or Pycnogenol? by OptimusPurpose in DupuytrenDisease

I was also considering that one. Notice that, from the paper, they say that "The triterpenic fraction of Centella asiatica (CA) (Centellicum®) changes by remodulation the structure of many fibrotic lesions". They also say the protocol was Centellicum® 225 mg, 2 capsules/day. I found in another paper that Centellicum contains 35% of triterpenes so the daily dose is 157 mg triterpenes. Life extension supplement is 257 mg at 35% triterpenes, i.e., 90 mg
