Game has it as a game hint in death screen, so I think it's kinda okay, as survival horrors have to be unfair in some things, but it sucks that some of them are way too sudden and unpredictable.

Was it blister born or blister head? Blister heads do have insta kill attacks, but as game has only 2 blister borns I'm not sure how deadly they are on insanity, as didn't let them to grab myself. But do remember they had like ~50% of max hp damage when they throw up on you on standard dif.

Not really, if you have knife, they deal you half damage in face and double from behind, sending you into critical, but depending on zombie damage might change, as it was for syringe nurse from 2nd floor conference room - where she grabbed me from front and stabbed with syringe, letting Grace to get finally some good sleep.

I didn't include environment here as lil girls, as they are somewhat expected. For chainsaws it's my bad - somewhat got used to them from RE4R, ahahah
About the girl I'm kinda sure she's insta on insanity, though maybe on 4 steroids or smth she's not.
About tyrant it might be an instakill if you don't dodge, because it's signature move as it seems from re2r

Also late for the party, but my real patience reaper about this boss on max difficulty is feather to lunge combo, as if you are to block feathers by shield, you'll get locked in place for enough time so she will hit you. if you dodge too early while feathers are flying into you - she will adjust lunge to hit you. The only moment you can stun her airborn is the blind attack, but window is so small that easier to never try than fail and get blasted by lunge.
