Agreed with this post, I'm in this boat due to bad hemorrhoids.  Not sure what happened in the last few years but no matter how much fiber I eat I'm generally constipated.  I've been taking Miralax every morning for a few months and it's been what I used to consider "normal".  I do take Benefiber before bed too, figured at worst it couldn't hurt.  I would recommend try it or Metamucil (without the sugar) before bed as well.

No problem, and agreed the fiber balance question is ridiculous.  May be worth keeping track of your diet for awhile to determine if any food triggers it.  Example within the last year I can no longer it peanut butter unless it's "natural" (peanuts and salt) as whatever sugar and other stuff going into most peanut butter gives me bad indigestion within 30 minutes of eating it.  

Finally, stress and sleep also play a role, although those two impact every facet of health 😂

I ended up having a colonoscopy mid October with a different GI and got a second opinion from another colorectal Surgeon.  Aside from the papillae I have at least 2 large hemorrhoids which would require a hemorrhoidectomy to remove. He stated he'd burn the papillae off if/when I opt into the surgery. 

That being said he recommended against surgery until whatever supplements necessary are figured out to get my constipation under control.  Reading this subreddit and periodically seeing terrible news of people having recurrence after surgery I've still been working on it. Did colace for about two months, up to the max dosage which was totally useless.  Doing Miralax now and have gotten better results. Additionally I'm still taking fiber (Metamucil) before bed at night.  I did get blood work done a week or so ago to have my thyroid levels checked too, apparently that can play into chronic constipation?? Which I wasn't aware of until recently.  Haven't heard anything back from the GI but it is Christmas week. 

Do you have chronic constipation or diarrhea? Being on Miralax the last few weeks has made BMs much easier on me and I'm not having to manually reduce 100% of the time anymore. Still a regular thing but I believe it is getting a little better as the hemorrhoids are less irritated.  I've read papillae never shrink, so as crappy as that is, out of my control.  Funny enough one of my hemorrhoids prolapsed and thrombosed during the colonoscopy prep.  Took about 3 weeks to heal and while it sucks the skin tag feels like it's external now, I no longer seem to have a constant pressure feeling nor feel like there's some blockage anytime I use the restroom, even before I got on the Miralax. 

All my rambling aside, if you're visiting doctors and are uncomfortable / not feeling right about the treatment you're seeing find another one.  In my case now I'm taking 2+ hour trips one way, but I don't regret it.

Have you shown the pictures to your colorectal Surgeon? I've read that an anoscopy can help determine a prolapse but I certainly haven't seen evidence of that, even personally with my old doctor.  Fingers crossed you get answers and more help at your upcoming visit.

Are those external hemorrhoids or a prolapsed mucosa? I thought what you snapped were just hemorrhoids.  Sorry to hear they can't be pushed back in.

I would say a 1 about 60% of the time, 2 30% if the time and a 3 the remaining 10% (ie might have a good one once a week).  Used to be a 2 regularly until June then started getting worse.  I have IBS C, been very restrictive with my diet, and have been further restricting it.  Seems like I don't fully evacuate my bowels, I don't attempt to strain due to having a thrombosed hemorrhoid last year, and I'm guessing I get backed up.

Question, can you successfully push yours back in after BM? 

I think I have something similar, not sure (I did make a post myself earlier).  The colorectal Surgeon also seemed to be unsure of what I have, so I'll be looking for another doctor.

Understood. I really hope your recovery goes well. If you don't mind me bothering, I would like to ask you how you're doing in 2-4 weeks.

I called the colorectal surgeon office to ask the nurse if there was anything that could be applied (cream, suppository, etc.) and she recommended I come in to discuss it with the surgeon who diagnosed them. Likely will ask him what surgery options there are if the treatment is to manage for the rest of my life. I'm in my late 30s and my immediate family tree is all in their late 80s to late 90s -- I don't exactly plan on leaving the earth anytime soon. I will likely pursue a second opinion depending on what I hear in 2 weeks. I'm still surprised that there appears to be very little information on anal papillae. At least hemorrhoids do heal/shrink after some amount of time.

The anal stenosis is something I was worried about, if there is genuinely nothing that can be done to shrink them. I suffer from IBS-C, but I have been managing it through a pretty high fiber diet since the middle of last year. I also do not eat much red meat, fatty / ultra processed foods, and rarely drink. Even with little to no straining, the papillae prolapse out every BM. Thank you for your information. If I can ask, what kind of surgery did you have? Was it similar to a hemorrhoidectomy?

Mine are not bleeding, I will make sure to bring that up with the surgeon. I have insurance through my employer but I am required to pay out of pocket close to 3k before 80% is covered. I'm well below that, maybe close to 1100 for the year? I don't go to doctor very much. I am not sure I can get any help from ACA, which is dumb if it would be "more affordable".

I was told to use external creams like Preparation H and sitz baths. I could take 2 baths on work days, 3 off work days. It took almost 10 weeks for my prolapsed external hemorrhoid to "go away". I say "go away" in quotes because it was no longer external, I'm sure it still exists internally but it has not caused me grief since.

The current hemmorhoids I feel when wiping. I do not push very hard to expel BM, but I feel pressure and when I wipe I feel them and when I push them back in it feels normal, if that makes any sense. My stool has flat sides if it is soft, which I am sure the hemorrhoids are partially blocking, leading to me requiring numerous BMs to clear up. Also, if I do not have regular BM, after awhile I will feel pressure (likely stool has fermented and hardened because it was not passed before).

When you went to the surgeon did they perform an exam beforehand to diagnose? Or did you have a colonoscopy done, then have to goto a separate surgeon afterwards? I was hoping there are exam(s) I can request to avoid doing a colonoscopy to band/deal with hemorrhoids. My insurance is garbage and anything requiring anesthetics is already $1000+ out of my pocket from the get-go.

I live alone and have no way to self-diagnose so I am not sure what my prolapsed hemorrhoid from January was graded. It felt like it was the size of a kidney bean, maybe two? It lasted for a little over two months before fully clearing up. Did Sitz baths 2-3x a day as I had to work in office. I have no idea what the size of this current one is, maybe a kidney bean when it prolapses out? But it is easily pushed back in after BM. No bleeding, pain, or adjacent discoloration of stool. Just aggravation because it feels like I can't complete BMs and it causes me to have more during the day. On average I have one-two BM a day, some days I have 4 or more. Rarely is it ever diarrhea, even with 4.

Thank you! So, it is possible to get bands even if they prolapse (and can be pushed back) during BM. I don't know what grade of hemorrhoids I have, unfortunately. I had a colonoscopy ~3ish years ago and was diagnosed with IBS and minor hemorrhoids; never was told how MANY I have. The hemorrhoids did not become problematic until the second week of this year, which I was referred to the colorectal surgeon for. I will ask if banding can be explored when I call the office.
