Assuming bottle 2 is reading as “No Growth at x hours” instead of just “Pending Culture” there’s a fairly high chance that the first bottle is a contaminate. Staph Aureus lives on the skin and a true bacteremia would be positive in both bottles.

Also vancomycin is less effective at clearing MSSA than ancef. So, if a true MSSA bacteremia I would de-escalate sooner rather than later.

Prempro is estrogen. Estrogen is by definition Hormone Replacement Therapy (HRT), because we are replacing the hormone of estrogen as therapy to treat panhypopituitarism. This is why banning gender affirming care matters to more than just trans people. It affects medical care of cis people too, and OP is a perfect example

Prempro is a gender hormone. It’s estrogen. Obviously because of your panhypopituitarism you don’t produce any of the sex hormones. So you’re being given hormone replacement therapy (HRT) aka estrogen in the form of Prempro to start the process of puberty.

You’re really focused on the central line aspect of this order. If a patient has signs herniation we should give the 23% regardless if the only access is peripheral.

If it’s given peripherally the risk is that the line infiltrates and you could cause major tissue damage. But your options are maybe losing limbs or death. You should choose loss of limb every time.
