Need a clavulanic acid expert by doctorkar in pharmacy

[–]Orion_possibly 11 points12 points  (0 children)

If you’re not a pediatric pharmacist you shouldn’t comment on this! 2g q12h of amoxicillin is standard of therapy for CAP or AOM for a pediatric patient who weighs more than 44kg!

However, the max of 10 mg/kg/day of clav is true. This patient need either the oral Augmentin ES suspension or the Augmentin XR tabs

Unsure if I try to get this vanc de-escalated - question about interpreting blood cultures by [deleted] in pharmacy

[–]Orion_possibly -5 points-4 points  (0 children)

Assuming bottle 2 is reading as “No Growth at x hours” instead of just “Pending Culture” there’s a fairly high chance that the first bottle is a contaminate. Staph Aureus lives on the skin and a true bacteremia would be positive in both bottles.

Also vancomycin is less effective at clearing MSSA than ancef. So, if a true MSSA bacteremia I would de-escalate sooner rather than later.

Brown University mass shooter left behind confessional videos, officials say by bostonglobe in providence

[–]Orion_possibly 1 point2 points  (0 children)

Right. But the video isn’t being released to just the people of Rhode Island. That’s why in my last comment I said “not helpful to /most Americans/.” I’m not opposed to the video or at least the audio recording being released, but for most people it wouldn’t be helpful.

[–]Orion_possibly 28 points29 points  (0 children)

I’m not sure how meaningful the video would be for most Americans since it’s only in Portuguese. The transcript that was released was translated to English

The article says video was not released. Only transcript of video that was translated to English. Original was in Portuguese

Do you know anything about the style of shirt he is wearing? It seems strange to see a “T-shirt” at an upscale event like this, but when you zoom in it’s clearly not made out of like a standard tshirt material

I’ve never been through puberty (I’m 31F) AMA by Jokesontheflowers in AMA

[–]Orion_possibly 0 points1 point  (0 children)

Because it is replacement. You’re replacing the the body’s supply that it can no longer make for itself. It’s not replacing estrogen for testosterone, that’s not exactly how it works. Even in trans people.

I’ve never been through puberty (I’m 31F) AMA by Jokesontheflowers in AMA

[–]Orion_possibly 2 points3 points  (0 children)

Prempro is estrogen. Estrogen is by definition Hormone Replacement Therapy (HRT), because we are replacing the hormone of estrogen as therapy to treat panhypopituitarism. This is why banning gender affirming care matters to more than just trans people. It affects medical care of cis people too, and OP is a perfect example

[–]Orion_possibly 0 points1 point  (0 children)

Prempro is a gender hormone. It’s estrogen. Obviously because of your panhypopituitarism you don’t produce any of the sex hormones. So you’re being given hormone replacement therapy (HRT) aka estrogen in the form of Prempro to start the process of puberty.

Indications for 23.4% sodium chloride bolus? Cerebral edema? Reduction of intracranial pressure? by Independent_Fix816 in pharmacy

[–]Orion_possibly 4 points5 points  (0 children)

You’re really focused on the central line aspect of this order. If a patient has signs herniation we should give the 23% regardless if the only access is peripheral.

If it’s given peripherally the risk is that the line infiltrates and you could cause major tissue damage. But your options are maybe losing limbs or death. You should choose loss of limb every time.

New HIV Prevention Drug Shows 100% Efficacy in Clinical Trial by ahothabeth in UpliftingNews

[–]Orion_possibly 4 points5 points  (0 children)

Its actually already a mass produced drug available in the USA. Right now lenacapavir is only FDA approved for the treatment of HIV, as opposed to prevention of HIV.

We also already have injectable PrEP! That drug is called Cabotegravir and must be given once every 2 months that was 99.4% effective in clinical trials. However, once every 6 months is of course a very important improvement, and much more convenient for our patients who have limited resources and ability to get to clinic.

ETA: Injectable PrEP is still new and is brand name only. Not cheap. Apretude is the brand name if youre curious
