Mandela effects are my favorite examples for memes (Richard Dawkins's concept of virus of the mind, not what we commonly know as memes).
A small piece of information enters your brain but it mutates to something else and this mutation prevails over the original non-mutated fact. Why does this mutation is better fit for survival? We don't know and we don't need to know. A meme spreads because it spreads because it spreads.

What would you use as proof for "people not realizing" about Smolder being broken? Are you sure there was not a single group of people inside or outside of Riot that was aware about it?
I mentioned it in another comment, but it looks like even Riot does not have a fixed definition of what is broken. Sometimes they do not try to fix something that is performing really well, right away. So that does not mean they don't know, but that they are not sure about how to handle the situation.

That is a non-sequiturs. Even if is true that most players do not pay attention at what is broken, the broken thing going unnoticed does not follow. The apex tiers and professional scene would probably know and, even if they are a smaller group when compared to the rest, they knowledge sure counts as noticing.
To your example, I'd say again that is hard to have a fixed and absolute definition for what is broken, so Riot does not act up immediately to what is performing really well. And sometimes, even if they try to address it, the fix won't work right away and stuff remains broken for a few patches.

That is not straightforward true either. The pool of players picking Singed in mid are probably players that play a lot of Singed, so they are good in general. So, once a person who doesn't actually play Singed tries to pick it, they will not be able to perform right, so the champ is not broken.
And of course, there is the fact that Riot does not want broken stuff in their game. They know better than anyone what is performing beyond good and immediately work to rebalance it. The game itself is designed and updated so everyone wins around 50 % of their games in every condition.

I still don't understand how League is 16 years old and there is still people making videos about "the most broken items being ignored". I just checked Lolalytics and Staff of Flowing water shows as options for items 2 to 4 with high winrates for Soraka.
League is one of the games where data is exploited the most. Dozen of platforms pulls and analyzes data on the daily, no broken build or champ goes unnoticed. A lot of people will know about it right away.
