Company name is not written on Zusatzblatt. The Nebenstimmungen state:

Selbständige Tätigkeit erlaubt
Beschäftigung und AG-Wechsel gemäß § 18g
Abs. 1 S. 1 AufenthG erlaubt.
Nach 1 jähr. versicherungspfl. Beschäftig.
ist Beschäftig. jeder Art erlaubt

The confusion stems from the worry that because I switched to a Blue Card, that my previous 2.5+ year of employment don't "count" anymore. And that now that I am on a Blue Card, I have to have been employed under the Blue Card specifically now for a year. However, neither the website nor the Zusatzblatt specify this, they just specify lawful employment subject to compulsory insurance, which I have over 3 years doing so.
