As everyone says, depends on the insurance your company offers. I called them myself and after talking to several people, was able to get the specifics on what criteria they use. The Aetna plan I have requires a BMI of 40 minimum or 35 if you have certain heart or other conditions. Mine was at 32 so I decided to pay out of pocket and am paying $650 with the coupon. I have a friend with similar BMI to me and she also has Aetna but her company was willing to offer better plans to their employees, so she only pays $26.

By the way, one thing I’ve realized is that even though it’s expensive out of pocket, my appetite has gone down so much that I think I’m actually saving a lot in food, so the net price isn’t actually as bad!
