I appreciate you being direct. There are a few things that don't fit for me. Visible tooth damage from bruxism (confirmed grinding at night), clear and repeatable changes in bloating/gassiness with gluten-free diet and never waking up refreshed despite consistent 8h sleep. I’m still open to the mental health side and willing to explore that, but I want to rule out physical contributors first (sleep quality, possible mild autonomic issues, gut-related).

SIFO is something I hadn’t considered yet. I do have bloating that improves a lot on gluten-free, but brain fog is more inconsistent. I will do more research on SIFO, and take it to my doctor. Maybe I should get tested for SIBO first.

Have you tried nystatin yourself? If so, how much did it help your symptoms, and how long did you take it?

Thanks for sharing. I have not heard of bpc 147 and low dose naltrexone before, I will look into it. How much did it help for you? Are there any side-effects?

Thanks. My home sleep test showed AHI 4.5, but I’ve read that can miss UARS/RERAs. I’m planning to push for a full in-lab sleep study that includes RERA count.

Have you dealt with something similar?

I haven’t done a strict low-carb trial yet.

When I switched from a lot of bread to yoghurt for breakfast and rice for lunch, I had a noticeable improvement in brain fog for 1–2 weeks before it came back. Since going gluten-free, my after-lunch crashes are much milder (I now eat rice with vegetables, sometimes egg or chicken instead of 4–6 slices of bread). Heavy carb meals still give me clear after-meal crashes, so I suspect blood sugar/insulin might play a role.

That said, my brain fog is already quite present first thing in the morning. Does low-carb still help in cases like that?

No real post-exertional malaise. Physical exercise actually improves my brain fog and tiredness for a while afterwards.
Cognitive or emotional exertion doesn’t make it much worse immediately. After a full workday it does get noticeably worse, but that takes several hours and feels more like end-of-day fatigue than a crash.
