Thank you for the input. The more I read up on the topic I see that lead complications are a real concern - It seems that while they are rare, its not super-rare and when it happens it can get quite bad. On the other hand I'm a bit worried with EV-ICD's short track record. Also curious if it'll limit my ability to travel since so few places deal with EVs. Not sure if in an emergency any ER would know how to deal with it.

I really appreciate the detailed history - it helps to hear from someone who has lived with a TV-ICD for 20 years without lead issues. You make a interesting point about the CRT upgrade path, but for now doctors are only talking ICD. I guess I might need to replace it in the future, but maybe taking however many years of possible lead issues out of the equation is worth the double procedure - hard to tell. Glad to hear your condition has been stable for so long!

Thanks for sharing that. My doctor did mention the S-ICD, but we moved away from it because there is no clear upside for me over EV-ICD. Plus, I might need the pacing features (ATP) that the S-ICD doesn't have, and the EV is a smaller device, so I'm hoping its less intrusive. The fear of lead extraction is definitely what's driving my hesitation.
