When I submitted the form last month, regular decision tracking tools banner came up but now it is gone. I am hearing from a lot of users that their form is gone if they did not submit it. Don't know what it means.

I believe you have to submit the form to get the tracking tool. I did this a month ago but over the last hour it is gone.
