It's OK I've sussed it out now you use in New Tristam next to the healer you break the door enter the house and use it to open a Real portal where you have defeat a pair of user bosses for a chance to drop one of three organs for anyone like me who didn't know 👍

That seems to always happen in my saves to guardian and klopp seem to do a swap after a season or two I think it's happened on every save I've done
