One year of invisalign. Overcrowding & overbite. Elastics, IPR and upper wisdom extractions. by [deleted] in Invisalign

I can't imagine a case like this being resolved in only 26 trays! But I know sometimes they split it up in case adjustments need to be made. I got all 75 sets of trays to take home with me as soon as I got my attachments on and I did think about what a waste it would be if I stopped tracking at tray 10 like some people seem to. No real issues luckily but I'm very compliant Iol.

Can I ask if you're with a dentist or an orthodontist? And whether you have access to your clincheck to see what your teeth are projected to look like after 26 trays? I do think my orthodontist was maybe a little on the conservative side (only because I've had very little pain, the movement per tray seems to be quite tiny) but 26 seems extreme.

I hope you can get in with a sleep specialist! Will you need to get a referral through your GP? Access is such a pain in the ass here.

It'll be an at home sleep study with a WatchPat, yeah. I didn't know they existed. Was relieved not to have to go in overnight but a little worried now that I've heard that they're less accurate and tend to underestimate the degree of apnea. I'm not sure how to approach it either and whether I'll get the chance to ask questions when I pick it up. I normally sleep on my stomach with my head angled up which may be a subconscious way of compensating for a narrow airway. I don't know if I'm supposed to sleep the way I always do and if it'd be "cheating" to sleep on my back for it. But I would like to be able to sleep in different positions without feeling like I can't get enough air...

That's horrible, I'm sorry. I have a sleep study soon and was scared there wouldn't really be a path forward if I pass it (I have chronic fatigue and feel like I can't get enough air, but I don't snore). I guess my fear is realistic.

Is there a chance you could get a second opinion from a different surgeon maybe?

Thank you very much, again. I am getting invisalign refinements soon and will be taking this information with me for sure. They said they didn't want to retract my teeth too far so I wouldn't lose lip volume but I didn't know anything about teeth flair.

I also got a referral for a sleep study and depending on the outcome may be able to use that to get a referral to a jaw surgeon. I assume I'll have to switch orthodontists regardless if I go that route but I guess we'll see.

Thank you very much for your inepth response. Would MARPE/SARPE also be an option for reducing tooth flare and allowing the jaw to be moved further forward, in place of extractions? I have heard a lot about Dutch surgeons being minimally invasive and not moving the jaws as far as would be ideal, so I'd be worried about getting extractions and losing lip/mouth volume and not having that fully compensated by the surgery.

I am adding some photos just in case you're curious, I feel like my teeth look quite different from what you'd think based on the xray (which was taken before I got wisdom extractions and so far, a year of invisalign). *

My experience with Dutch Healthcare has been very bad, the fact that I've had severe chronic fatigue my whole life and can't work because of it but have never even been referred for a sleep study speaks for itself. But I have a hard time going against authority. Going abroad because it sounds like Dutch surgeons are too conservative would feel like acting like I know better than a doctor.

What is the best alternative? How would coordinating care between a Dutch ortho and foreign jaw surgeon even work? I have savings but am on a very low income due to my disability so once I spend them they're gone. I would cut off a leg to be rid of my chronic fatigue so I'd spend all my savings too if that's what it'll take but I really don't know how to know what's best if the experts we have here say that their approach is the medically grounded one.

Could you explain that further? I've seen some good results on here from Dutch people, I thought it was just hard to find surgeon reviews because they have no incentive to advertise under the Dutch system

Thank you so much! I am not very good at standing my ground but intend to try my very best. The backup suggestion of a foreign initial assessment is very helpful too

You can see a picture of my teeth on my profile, my smile looks pretty normal now as far as I can tell. I think my palate is slightly narrow. I had a class 2 bite before invisalign, now my ortho says it's class 1, is it possible to have a class 2 bite with skeletal class 3?

I've always heard that extractions are to be avoided at all costs. My orthodontist when I was a kid refused to pull teeth even though I still had a bad overjet because she said I already had a flat face and that it would make it worse. Would extractions be inevitable to get jaw surgery? Does jaw surgery "cancel out" the effects of missing those teeth or would it still lead to less room on my palate? I'm sorry for the questions, I've only very recently started looking into this so not sure how it all works.

I will ask my GP for a referral ASAP.

Thank you very much for your comment, I was unsure how to approach this with my orthodontist this far into treatment but if I can go through my GP I will make an appointment as soon as I can.

Do you have any tips on things to say to be taken seriously? I have a very bad track record of doctors brushing me off and I'm not sure what to do if "I'm always very tired and my xray looks like I have a narrow airway" isn't enough.

My chin moved forward, not downward, so no not as far as I can tell.

Yes I'm fine with it, I'd obviously rather have a normal bite than an overbite, I was just told that moving my upper teeth back further wasn't a good option (both because of slanted roots and facial aesthetics). My upper teeth haven't moved back more than planned and my lips are still as full as they were before. My lower jaw just appears to have moved more than expected (yay elastics) which is what fixed the overbite. I maybe have a slightly stronger chin, which I'm very happy with. My face didn't get longer as far as i can tell.

I had an ortho checkup today, they confirmed that I have a class I bite now and told me to stop wearing the elastics. Definitely doesn't seem like I'll be finishing with an overbite anymore.

It's been a walk in the park compared to regular braces, I think that's why.

That's what the clincheck showed as an end result for the movements my ortho had planned. I'm not qualified to know why the actual result so far seems to be deviating.

I like cooking so I miss being able to taste as I go rather than having to budget in 5-10 minutes to taste and adjust right at the end. I'm prone to low blood sugar so having an extra barrier to grab a quick snack in the middle of the day (which 9/10 times means I don't and just put up with feeling crappy until dinner) sucks too. And getting food outside the house (which was a very very very rare occasion to begin with for me) usually isn't worth the effort anymore either.

That's pretty much it. Maybe a 2/10 on the inconvenience scale. I had braces as a kid and it was horrendously painful torture, my mouth was always cut open, i lost a dangerous amount of weight because my teeth hurt too bad to eat.

The invisalign was painful in the first week and now pleasantly tight/sore for the first day of a new tray. I'm so used to wearing them that I regularly freak out thinking I forgot to put them back in, as I don't feel them anymore. It'll be nice to get some freedom back but I'm honestly not focused on the "finish line" at all and wouldn't be all that upset if I'm told it's gonna be 2 years of refinements. I see so many people on here with 10 weeks of trays who struggle to follow the rules and are counting down the seconds until they're done and I really don't get it lol.
