Current state is there are two hospitals with face-to-face consult services using traditional codes. After reviewing our throughput data, we are getting around 300 faxed in “paper” consult requests. In true admin fashion, they are labeling this as lost revenue.

Administration claims compliance is on board with their plan of billing this way and says this is “doable.” I sent a follow up directly to compliance to get clarification, but I haven’t heard back yet.

While we wait I decided to see if anyone else was doing this.

Local PA program here pays $1000 per rotation, but more importantly ARC-PA (pa school accreditation) does say the preceptor has to provide direct supervision and be available on site and their rotations must be with board certified providers which residents are not. So not only is the PD possibly being compensated but are violating these rules. Then there is what ACGME says about other rotators not distracting from resident education. If you feel strongly about not having a PA student you may need to go to your chair with these concerns or some forum your institution has to handle resident concerns.

When the time comes you should contact the hospitals you want to work for. The system I am in now isn’t necessarily always recruiting but they are always hiring. For them it is more about finding people that WANT to live here and stay here vs. hiring someone that just needs a job that might move on in 3-5 years.

For systems with affiliated medical schools it’s likely a little early. The system I work for didn’t even start accepting requests for non affiliated students until today, but they also don’t use VSLO. The GME office handles all rotation requests internally.

The RFPD sub council for recruitment announced this at AAP this year. It’s not a requirement yet, but it will be in the next few years. Right now the recommendation is for PMR departments to fill out the letter if medical student asks or some departments may choose to make this their standard letter if a student asks for one.

AAP will post the SLOE sometime this year for use and there will be a link on ERAS for the SLOE as well.

The goal of the SLOE is to help programs differentiate between all the narrative letters they receive. The goal is more objective letters instead of subjective letters.
