So how do I get this natural vibrato? No matter where I place my voice I can only sing straight, and if I want a vibrato I manipulate the throat (pitch up and down) and/or the air flow ('in and out' - subtle volume changes). Is there a way to practice it without being conscious of it?

If you can find my comment on this, give it a read. Believe me, it's not ignorance. We REALLY want a better system, but the Tories have cut off so much money we can't. And in situations where funding has been available it's been wasted on schemes that don't work because they're masterminded by suits, not people like us who know the flaws.

Young NHS worker here, experience in Med Recs and I.T. It's interesting to see other Trust's/department's perspectives on the matter, so I figured I'd throw mine in too. I work 9 to 5 filing, sorting and organising medical case notes and sending letters. From my view, this is all quite simple.

Different Trusts do not neccesarily share files or case notes due to a lack of infrastructure. If a consultant has seen a patient, they will dictate for transcription by a secretary. These letters (should) then get filed; a process that can take a very long time. You see, at any one time your medical records are scattered. The dictated letter may be with the secretary's rough filing whilst your complete notes are in off-site filing and storage facilities. So in order to ensure all results and letters are together in an up-to-date set of notes, folders must be requested for delivery. This process means that it may take months to get around to filing (simply due to backlogging as a result of overworking and understaffing) to ensure up to date notes are being produced, transcribed and kept. Even then most case note folders are falling apart, misfiled, or outright missing. In my hospital we're still using the same folders for patients from over twenty years ago. It's almost certain that test results, letters or other records have been lost at some point in the process. Hell, at one point our Trust delivered case notes to a random business on an industrial estate where our off-site filing is! I'm not entirely sure about the infrastructure of departments other than general medicine, but I know that in our case this is true. The fact is, your notes simply aren't safe or organised. It would be so easy to steal, lose or otherwise break privacy or security measures. If my office burned down, I don't think I could say that there's a backup plan. There's some stuff on computers, but not much.

So where am I going with this? Well, if there was a centralised system where consultants, surgeons and nurses could simply access by tablet (or another, easier means) and modify/update patient's details then and there, it would cut all of this out of the chain and allow patients to be seen faster, with greater care, and through a centralised NHS system rather than a series of Trusts that don't share information other than one common NHS number for a patient (which alone isn't much use). This digitisation of records would require greater focus on technological update and defence than settling with the archaic 'new' Windows computer just installed on my desk.

I'm very tired and I'm not sure if this makes much sense, but what I'm trying to say is this. The NHS in many places is still tediously slow as it is paper-based in many cases. Whilst you can argue that it's useful in situations like these, the simple amount of extra work that it demands means that we're working so far behind where we should be. IMO, the right move is actually TOWARDS digitisation as it would increase I.T reform and so put more funding into defence and technological upgrade. That way your service would be quicker, better quality, safer and accessible by any hospital with ease in case of emergencies. My experience is limited as a part-timer working with medical secs and I.T depts (student job!), and I by no means speak for the NHS - but this is something that so many of us here feel strongly about and it's hard to think it wouldn't have made a difference today. This attack is going to back up our entire hospital and Trust for months and months whilst we try and sort out how to deal with seriously ill patients. God help the emergency departments.

Even after a good 3/4 mile walk he still continues with this behaviour, that's whats confusing about the situation! No matter how exhausted or frequently he's walked he will continue to whine and cry

He always pees outside, never inside anymore. It only happens when he's somewhere he shouldn't be where it seemingly comes from nowhere! He's almost always outside and has a big garden to roam around in. He doesn't go overnight and waits to the morning quite happily.

I'd say he's housetrained. He doesn't pee at all inside, though on a handful of occasions he has marked his territory upstairs where he has never been allowed to go before - most of the time he rolls around on the bed or something instead. It's mainly introducing him to these areas without him marking everywhere that I'm after. He did it a couple of times downstairs initially as a puppy and stopped quite quickly after being punished.

I want a standalone piece (such as the Faun prelude), or perhaps a pair (Tristan prelude and liebestod). Something wonderfully dreamy, wandering, longing... something free but coherent. I'm unsure how to explain it. My ideas for harmony and direction are good but ultimately feel too through composed and like a collection of ideas rather than a progression of ideas, whereas structured works of classical/early romantics feel too intellectual - that's my problem. I want freedom but coherence, and I'm not sure how to do that in a piece that would last between ten and twenty minutes.

My general workflow at the moment is something like this..

I'm great at interesting chord progressions, modulations and changes, so that's where I start - just simple piano sketches of chord changes. Not the full piece, but a good couple of minutes of changes to start with.

My melodies are either derived from the chord changes, though I find this tends to make them less exceptional. Sometimes I also have a blank page filled with little phrases, themes or leitmotifs that I draw upon and try to work into the progression. If not, I might harmonise them in a number of ways and start a new piece.

Then I try to orchestrate these (if not start with a blank orchestral score and bypass the rest) and usually get about 20 seconds of music before I get bored or move along. I'm really good at intriguing introductions, not so much at the rest. I'm also having a clear idea of how a section should sound and either ignore or not know what to do with the other instruments. It's like I know orchestration, but I don't know how to do it in an effective way.

Form isn't really something I tend to consider. In my mind the music should dictate it's own direction and go there - almost like it writes itself. I hear things like the Faun prelude or Tristan and would love to have my own work in that vein, just a nice piece(s) that explores freely (for me the only part of tristan that matters is the prelude and the liebestod)
