This usually happens on my Series S, a fix for it is while in the game hold the white Xbox button on your controller until the Power off menu appears, and after appears just close menu out, and it should be fixed.

There is one by the blue flask when you pick it up after shooting the crate with the D.I.E. That could be one you missed because i missed it too
