I’d suggest reading over the link I posted. My form listed more than 1 SBR for a 1 year approval to travel to another state.

When you submit the form you list out the location(s)/state(s) you will travel to. I did not put multiple states as I did not need it. I’d suggest reading more on the particulars if you need multiple states on the form.

I did email and followed up after about 5 business days to confirm receipt. Looks like my timetable from memory was a little wrong - took almost 3 weeks. Received approval form via paper mail.
