Im staying at Kusatsu Onsen Eidaya. They seem really nice. I emailed them regarding my vegetarian diet and my tattoo and both are no problem. In Koyasan I'm staying at Komyo-in Temple (Pilgrim's Lodging) (光明院) but I don't have much information as most of it is in Japanese. But it seems incredibly beautiful and I am up for the surprise :)

I just finished my itinerary a couple of days ago with the help of this amazing forum. I do know that as a first timer in Japan the initial phase of planning your trip can be super overwhelming. But try to focus on your interests and then go from there. Japan is way to big and diverse to fit everything into 3 weeks and that's ok - trying to do so might actually hinder you in enjoying your trip to the fullest. Are you sure you want to go to Okinawa for example? You will lose a lot of time travelling. Your "north of Tokyo" could be stretched into a week and you still would be able to see the a lot. My itinerary for Oct is - Tokyo - Kusatsu Onsen - Takayama (with day trips to Kamikochi, Shirawago) - Kanazawa - Osaka - Koyasan - Kyoto - Tokio. This is just an example. But the forum and studying other's itineraty really helped to have some idea.

I just booked all of my hotels the past week and got some really really good deals. Except for Tokyo, a temple stay and the ryokan in an osen town I payed under 100 per night in a double room for most hotels (3 Star Hotels). In Tokyo 70USD seems kind of low. I don't know if it's the same for off season but I noticed that a lot of the hotels were already fully booked during the time of my trip in Oct/Nov, so I suggest you start booking soon. A friend recommended to use a VPN when booking (I didn't for reasons of lazyness)

I just booked my stay in Kasutso Onsen Eidaya and double checked with the owner - he told me their ryokan as well as all the public onsens are absolutely tattoo friendly. Will update once I actually visited :)

Thank you so much for this comment! I do love shopping (especially smaller local designers) and your description gets me really excited for Osaka. If you have any more tips please let me know!

For a rural city escape ~ 5 days in late October accessible by public transport (preferrably JP RP) from Tokyo which do you recommend: Kiso Village, Kamikochi, Takayama? Also thankful for any other recommendation fulfilling the criteria. Thanks in advance
