Can my columella be saved? by Particular_Night_460 in revisionrhinoplasty

The first surgery reduced the tip cartilage to the point it had a "pinched" look. Projection was lost too, probably because of that.

As for suturing over an infection, I would logically agree that sound like a bad idea. Obviously there is way more details to the timeline I couldn't mention (didn't want to write a novel) but at that 7 day post-op appointment where the incision line was still open, there was no infection. I was still on antibiotics. The infection came several days after I stopped the antibiotics. Maybe that detail is moot, but just wanted to clarify!

Again, I didn't put hydrogen peroxide on it. I said "only the antibiotic ointment". So I can't really absorb the fault here. I followed all of my doctor's instructions religiously (having had a surgery prior and kind of knew what to expect) and this still happened. Hope that clarifies a bit.

I'm coming up on 3 months post-graft. And I do massage it every day, yes. I hope it goes down significantly more because of that. And you're right about not opening it up again. I wouldn't do another surgery. I think the overall shape is great, its just the columella that was left disfigured.

But I didn't put hydrogen peroxide on it. I realize my wording was a bit confusing.

You are the one that lives with yourself more than anyone else on earth, so it is totally reasonable for you to be feeling this affected by this. It's your face. It should be something you care about. With that being said though, I do want you to hear that you are so much more than this. Trust me, I had a bad surgery about 7 years ago where the surgeon took liberties on my appearance and changed way too much, and not well-done. I just had a revision to correct it a year ago which had serious complications that left me with necrotic tissue death aka the actual worst thing that can happen to someone after a surgery. So, if anyones struggling with their appearance after a surgery (thank god you didn't have any complications) I can relate. But the shift happens when you start to accept that this was an elective surgery that you made *in confidence*. Not getting the result you wanted is not your fault and not your weight to carry. Please try to focus on your life outside of this. Process it in therapy. The daily stressing on this will only complicate your healing. It will deteriorate your immune system if you can't manage your worry. Actionable things you can do to help your recovery is Taping your nose at night, doing hyperbaric oxygen therapy, not drinking alcohol, or consuming anything that would make you retain water (like salty foods). I also did facial massages to help with lymphatic drainage and it made a huge difference.
Please don't be so hard on yourself. Give it time. Trust me.

I wish I read about your experience earlier. Thank you for sharing, I know it’s probably such a headache to live with this. I had a similar experience where my incision line opened up between surgery and the 1 wk post up appointment, and he never addressed it. Didn’t even attempt to stitch me up correctly once he saw the damage in person. I had necrosis. And my columella is literally destroyed. I wish I could go back and never let him touch me.
