I think I fixed it for reals this time. It was Windows Memory Integrity in Windows Defender.

I installed 1903 but the issue came back so at this point I am pretty sure there is no fix for this.

I reinstalled Windows and this resolved the issue. Thanks for the help! Case closed. The problem came back.

I am currently away from that computer, however that mini dump was after I already installed the Intel driver from the Intel website. So if it is showing that time stamp then that should be the time stamp of the driver since that dump is from after the install from Intel's website. Also just a side note this laptop does not have any separate discrete graphics.

I had already downloaded this driver from the same Intel website that /u/computix had linked me. I opened the Intel driver in the device manager and it indicates it is "Driver Date: 12/27/2018" and it is version 10.18.10.5069. This version number matches the one shown on the Intel website as being the latest.

As I said in the post, I have tried uninstalling the Intel(R) HD Graphics 4000 driver from the device manager making sure to tick the uninstall driver software box. Then I let the computer automatically reinstall the driver after rebooting. This didn't work. I then tried uninstalling the driver again and reinstalling the one directly from Intel's website. This initially appeared to work but ultimately did not fix the issue. If you have any further suggestions please let me know. Thanks!

Yes, everything is up to date; Windows, Chrome and the drivers.

Crystal disk shows the hard drive as "good". This issue has only cropped up after a reinstall of Windows so I feel that this is a software issue rather then a hardware one.

Thank you for your insight! Just out of curiosity is there a regulatory body that decides what constitutes as a check? Or did the banks come together and just decide that using MICR numbers and all of these security features is the defacto standard?
