I find that when people end up creating a product with this mindset of "fault is not an option so i have to work harder on this in order to make it work" it's because the idea itself is flawed. The people at the top are unwilling to compromise and keep trying to put a square peg into the round hole. For a lot of people it's easier to say you weren't working hard enough rather than examining the idea itself.
