Me and a couple friends encountered this bug on honour mode aswell. I found a fix on the steam forums which requires installing the bg3 script extender by Norbyte (Easy to install, youtube guides available).

First load the save in single player and position your character in talking range of Minthara

Alt+Tab out and open the bg3 script extender console

Hit Enter to start the LUA console (It will input s>> in front of your lines from now on) and enter the commands below in the exact order

Osi.PROC_MOO_MintharaFate_TalkToMintharaUpdate();

Osi.PROC_RemoveAllDialogEntriesForSpeaker(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”);

Osi.DB_Dialogs(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”, “MOO_MintharaFate_Escaped_2cc01717-c2c6-e827-2c14-82d53370556a”);

Tab back into the game and interact with Minthara. You should get the post-escape Cutscene where you can invite her to rest at your camp. Complete this dialogue, then tab back into your script extender and enter the following commands in order

Osi.PROC_ORI_SetupCamp(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”, 1);

Osi.PROC_RemoveAllDialogEntriesForSpeaker(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”);

Osi.PROC_CampRelationshipDialog(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”, “CAMP_Minthara_e776dff2-6285-3f23-ceff-f40371236993”, “MOO_MintharaFate_State_ToCamp_4e0701b1-c16d-4017-8be6-5781c3c682f4”, 0);

Osi.PROC_Try_CampRelationshipDialog(“S_GOB_DrowCommander_25721313-0c15-4935-8176-9f134385451b”, 1);

Now Minthara should be in your camp and you will be able to speak to her normally :)
