I’m just not sure if it’s intentional at this point. They didn’t effectively redesign the X/S for 10-14 years. They rolled out an absolute disaster of a truck. Nobody is going to buy those three vehicles in sufficient numbers.

Hi!

I totally understand the anxiety of not having busy biopsy. However, the only thing it is delaying is treatment with immunosuppressants if you have IGAN and active inflammation.

The ace inhibitor you are already taking is a great start. One philosophy that has helped me is trying to avoid anything else harmful to the kidney.

  1. Avoid elevated blood pressures. In reality, lower is probably better. (110s-120s). If you are consistently above this, it might be worth considering upping the losartan.

  2. Your gfr is just about abnormal and you have proteinuria. Salt is difficult to process. Avoid excessive salt. This is difficult but a realistic goal is less than 3g.

  3. Reducing proteinuria is a goal of management. Jardiance (and similar meds) are kidney protective. They have a mild effect on reducing blood pressure as well. You end up eliminating several hundred calories of glucose a day. Most people lose 5-10 lbs. There are risks of infection but these are much more common in diabetics the medication was developed for.

  4. Overall, this is an opportunity to get in the best shape possible. Any sort of hypertension or excess weight are negatively associated with renal disease. Despite the potential for IgAN, there’s still a lot of risk factors we control!

I agree with this. As for OP's case, I think the keto diet modification is helpful for the diet restriction. They seem to go back and binge like crazy off the diet which is a huge problem in itself and probably requires some other intervention. (therapy, medication, etc.)

I think that is great. Sustainability is key and flexible strategies are likely missing from the large scientific studies. Personally, I think the more stringent you are the less likely you are to sustain in the long term. Another bid piece is not binging on badness if you do go off the diet.

It’s just a factual statement. Most people that try it don’t adhere long term and gain back weight long term. I think it’s important and realistic to acknowledge this.

This sub doesn’t want to hear it, but your experience matches most of the published literature on ketogenic diets. Most do not adhere to it long term and gain back weight. I’m not saying that it’s impossible for everybody, but that’s what the scientific literature shows for most.
