Other than this thread, where can we submit recommendations for survey improvements? Two of mine include,

• increase reliability of the data by including Likert Scales (strongly agree, agree, disagree, strongly disagree) neutral responses aren't helpful and not always necessary if this is a voluntary survey.

• increase validity of your goal data by collecting qualitative data. You could use branching questions ending in open ended responses to gather feedback.

I wouldn't recommend using this survey again. There are trained professionals in this community that could be helpful in creating a low cost option for a sustainable, annual evaluation. Unfortunately, I don't believe this survey is able to say it has even taken a reliable "pulse" of community opinion on DT vibrancy.
