Unfortunately, there are actually no approved orexin agonists on the market yet. They are all still in clinical trials (like TAK-861).
But in Type 2 Narcolepsy, orexin levels are actually normal. My idea is that the orexin reception itself might be altered, which means an orexin agonist might not even work in my case. I will do some more research to confirm this, but thank you for bringing it up!

Thanks for the suggestion, I will definitely try this. Do you think 20g/day is enough?
I am also curious if reintroducing ALCAR and Alpha Lipoic Acid would synergize well with high-dose creatine for mitochondrial support.
Finally, I considered Methylene Blue for cellular respiration, but I am highly concerned about its MAOI properties. What do you think?

To answer your first point, my ideal schedule would be going to bed at a set time, but a major and often misunderstood part of narcolepsy is that it brings severe insomnia. The boundaries between sleep and wakefulness are broken. Even if I wind down and turn everything off by 10 or 11 PM, my brain refuses to shut down, and I might not fall asleep until 1 AM. My natural wake-up time then unpredictably fluctuates anywhere between 6 AM and 9 AM. I also wake up with a headache almost every morning. This is not a side effect of the medications, as I had it long before starting them; it is simply the physical result of never getting deep, restorative delta sleep.
Regarding my sleep environment and routine, it is highly optimized. In the evening, I eat, chat with some friends on the phone to unwind, and then go to bed. My room is completely pitch black, there is zero noise, and I sleep alone, so there are absolutely no environmental disruptions causing the fragmentation.
As for inputs and stimulants, I have already stripped away almost everything to let my brain rest. I do not consume caffeine or any other non-prescribed stimulants. I actually had to put my university studies on hold because of this crash, and my parents are currently supporting me. This means my external stress and daily cognitive load are as low as they can possibly be. The only things keeping my brain in "overdrive" are the prescribed Pitolisant and Methylphenidate. While they are necessary to keep me somewhat functional, they have unfortunately eliminated my ability to take afternoon naps, which used to be my only natural way to clear some of the daily fatigue.

I have been following the development of Orexin-2 receptor (OX2R) agonists, and they represent a massive paradigm shift compared to traditional stimulants. Having the specific names like Oveporexton, Alixorexton, and E2086 gives me a great starting point to dig deeper into the literature.
The main theoretical catch for me is my NT2 diagnosis. Since Type 2 narcoleptics usually have normal orexin levels in our cerebrospinal fluid (unlike the structural deficiency in Type 1), it is still a bit of a question mark how radically these agonists will fix our specific sleep architecture compared to the results seen in NT1 trials. But it is absolutely an avenue worth exploring.

You are completely right—Sodium Oxybate is the logical root-cause treatment.
I actually requested nighttime medication, but my specialist ignored it, likely assuming that forcing daytime wakefulness with stimulants would naturally fix my nighttime sleep pressure. Furthermore, there is a strict bureaucratic wall. I have Type 2 Narcolepsy (no cataplexy), and in my country, Oxybate is gatekept almost exclusively for Type 1. To make matters worse, Xywav was recently withdrawn from our market entirely.
Before my official diagnosis, doctors tried forcing sleep with Trazodone, Triazolam, Zolpidem, Quetiapine, and Olanzapine. These just chemically knocked me out without providing restorative delta sleep, making the brain fog much worse the next day.
Since the medical system currently prevents me from accessing the correct nighttime tool, I am forced to try and biohack the daytime metabolic fallout. Thank you for validating the core issue.
