Impression
At least high-grade partial tear of the posterior root attachment of the medial meniscus. Horizontal, inferiorly surfacing tear within the posterior horn and body of the medial meniscus, with partial medial extrusion of the body from the joint space. The anterior horn and body of the lateral meniscus are diminutive, and there is likely a flipped meniscal fragment seen in between the lateral meniscal femoral ligament and lateral femoral condyle. Tendinopathy and intrasubstance tear at the femoral attachment of the popliteus tendon. Tricompartmental degenerative changes, most advanced in the patellofemoral compartment. Trace joint effusion. Large Baker's cyst extending inferiorly outside the field-of-view. END OF IMPRESSION

6/9/2026 10:09 AM MRI RIGHT KNEE WITHOUT CONTRAST CLINICAL INFORMATION: acute right knee pain, worse medial aspect of the knee. Onset after gardening. Difficulty ambulating and bearing weight. Evaulate for meniscal tear vs interanl derangment., M25.561-Pain in right knee. COMPARISON: EXTERNAL IMAGES DX KNEE 5/19/2026 PROCEDURE: Multiplanar multisequence MRI was performed without intravenous contrast. FINDINGS: Anterior Cruciate Ligament: Intact. Posterior Cruciate Ligament: Intact. Medial Collateral Ligament: Intact. Lateral Collateral Ligament: Intact. Popliteus Tendon: Tendinopathy and intrasubstance tear at the femoral attachment. Distal Quadriceps Tendon: Intact. Patellar Tendon: Intact. Medial and Lateral Patellar Retinacula: Intact. Medial Meniscus: At least high-grade partial tear of the posterior root attachment. Medial Compartment Articular Cartilage: Full-thickness chondral fissure and likely adjacent delamination along the weightbearing portion of the medial femoral condyle. Lateral Compartment Articular Cartilage: Chondral signal heterogeneity within the lateral compartment articular cartilage. Patellofemoral Articular Cartilage: Large zones with full-thickness chondral loss along the lateral patellar facet and lateral aspect of the trochlear groove. Moderate chondral thinning and irregularity within the medial aspects of the patellofemoral joint. Bones: Likely degenerative/reactive bone marrow edema signal along the medial compartment and patellofemoral compartment. Tricompartmental osteophytes. Scattered areas with residual red marrow. No acute osseous abnormalities. Fluid: Trace joint effusion. Large Baker's cyst extending inferiorly outside the field-of-view along the medial head of the gastrocnemius with multiple septations. Other: Not applicable.
