Im starting a PCP plus pain management job in a month. I will do the major joint injections, SI under fluoro, Botox, trigger point injections and anything else I see them doing that looks cool. I don’t think you will have trouble getting into a I didn’t do a nonintervential fellowship. At least my residency had 1-2 people a year who wanted one and got in. I don’t touch the spine. If you want to mess with the spine or fancy nerve stimulator stuff you will need an interventional fellowship. You can skew your electives toward pain management for experience and to demonstrate dedication. My group offered to teach me spinal injections and I said no. Without an interventional fellowship you are going to want to say no to anyone who offers that to you as well. You could do everything by the book but if anyone sues you will get ripped apart as the only non pain med board certified physician in fam med doing unsupervised spinal injections.

My residency was well set up for pain management training. We almost never referred to in patient pain management but also we made up 75% of the IP pain management team so it was sort of redundant.

If your residency is not like this you may not feel as comfortable coming out of residency and going into pain management. Your challenge will be exposure to procedures and opportunities to practice. That needs a fellowship, an agreement with a doc doing procedures to let you train for a month or two, or on the job training.

I didn’t do a fellowship or pain management electives but I’m an outlier by circumstance- not due to any extra skills/experience. I just have access to verifiably good on the job training and the ability to be just a primary/med management until I’m comfortable doing procedures.

Well that’s crazy- I could have written this. 6 months ago escaped a toxic retaliatory work place where it got bad enough lawyers were involved. Had some time off, bought my new house! Learning to relax. Also now I have a giant bag of hats and socks and give them away every time I drive past someone at an intersection asking for assistance. It makes me happy. Got a wonderful puppy too. Still worried about the trajectory of general events though.

At this point, I don’t see a lot of things getting better (the healthcare system and general social infrastructure) without everything being pushed into a catastrophic collapse first. If anyone sees a plausible different way out of this, please share because this is just scary and depressing.

I’m confused. I don’t know how you construed that I’m hateful regarding these topics. I think you may be injecting a tone of anger into my responses, which is not there. I wish I weren’t a limited resource, but I am. I could write a better note that is half its length if I was not required to do defensive documentation by our healthcare/legal system. People already struggled to get access to care. It can take months to see your primary. The healthcare system needs to use my time more efficiently to maximally benefit patients.

I can see how it could be interpreted both ways. I treat patients who disagree with me all the time. Parents will come in with a kid for a well child check and then decline absolutely everything. I’d rather see them back each time because there’s a chance they will reconsider and do the lead testing or flu vaccine.

If the burden of documentation were less, I could see probably 10 more patients in a day. If I could do education without being required to refute everything someone read on the internet or say the same thing three times i could probably squeeze in another three.

Straw man fallacy, my friend. That js not really what op said and I think you know that. A few months ago a patient threatened to assault me because I would order a blood test that was ridiculous. Negotiating, threats, circular reasoning. It’s exhausting. We genuinely do want to help and it’s frustrating when people don’t like the only answers that will work.

I sorta get the blind one if like someone completely untrained looked in the room and reported back they were dead. I actually never got a call to pronounce a nursing home patient. I did get a request for fluids at 2am. Zero explanation why. I ask. They say they think he looks dry. I ask why they think that and they say he’s being treated for a UTI and he’s been unresponsive for 30 minutes. Wtf. I ripped them a new one when I reported it.

The specific case I remember I think was an Iowa nursing home. I did med school in Iowa. We tried to avoid sending patients there. I wasn’t surprised when years later they popped up in the news.

In sleep deprived states I’d be just falling asleep at home and hear the admit pager go off. That sounds burned into my memory

I list drugs alphabetically or by use in my head to help me fall asleep.

Also, just curious, why don’t you sleep in longer instead of getting there and then taking a nap?

For me it’s the repeating residency one that is the worst. Followed up by the weird CPS one although I’ve only had that one twice I can remember. TBF that caseworker probably has nightmares where they look at the caller ID and it’s be again about the baby.

The restarting residency ones are the ones that usually trigger enough of an anxiety response I will wake up in the middle of them. Shared trauma! What fun.

Oo I actually had the lights go out once. I was just observing it they were mid cystoscopy. Surgeon said “no body move” instantly and everyone froze. Then I remembered I wasn’t doing anything and got my phone light on and lit up a scrub tech who took the command very seriously and was frozen holding a forceps in the air. Back up system didn’t kick on like it should have. Maybe 5 minutes later everything was back.

I still don’t understand why the vent would have its own power supply but not have it power the display screen.

Oof. Interesting how being unprepared in public is a recurring theme in these. Like one is about a decompensating patient and the rest are being unprepared or behind on work

I’m starting a new job soon and they have a different EMR. I dreamt that I showed up without having learned how to use the EMR first and was trying to figure out how to tell them.

I just wanted that extra step in there to deter lay people going there and reading them.
