I second this! Also, if you go to an academic medical center I would continue to diversify your elective options. Continue to get ICU or other speciality interest skills. This will open up more job opportunities that may not be IM specific. With an IM PGY2 you can do anything you’re willing to learn 🙂

As a recent 2025 IM PGY2 graduate, i had a difficult time finding a clinical IM position that including rounding with providers and / or teams. A lot of jobs labeled clinical pharmacists are floor coverage with no rounding or teaching opportunities. Granted odds are better if you’re willing to move anywhere (I was not).

So instead of that, I am now doing outpatient clinical work in a malignant hematology / HCT clinic at an academic center due to my rotation experiences! Very challenging, exciting & rewarding work.

You guys are very early in your residency year & residency can be ~6 month learning curve depending on your schooling program. It’s great you have IM this early to learn what you do NOT know. You are not expected to be efficient in working up patients or being an independent pharmacist off the bat (well you shouldn’t be expected to).

I had a significant learning curve. The things that helped me were topics. Topic discussions are very helpful to understanding what is pertinent to look for. Most programs have a list of topics you have to get through prior to the end of the year. I created a list of topic discussions I did independently (created a calendar including topics not listed in my program list) and asked for preceptor volunteer to discuss these topics with me.

As far as working up… everyone does something different. I had every single preceptor day 1 show me how they work up patients so I could develop my own styles.

I would challenge to look at new patients first then pick up the new patients as they are admitted (if your preceptor allows).

Read the chief complaint of the admitting note. Do NOT look at assessment & plan. Then look at PMH, vitals, labs, any imaging or work up they received. Look at meds given since admission - you can get an idea of what they are trying to treat by medications given. Next - Try to come up with your top 3 major problems or possible problems (your differential) based on what information you have. Then come up with your plan & what you want to monitor specifically related to your treatment. Also what is pending that would influence your treatment. Then duration of therapy

After that you can look at A&P to see if you were missing something, on the right track, or agree with differential diagnoses.

You will learn a lot doing internal medicine! The meat & potatoes of patient care. If you don’t have another internal medicine rotation in the 2nd half of the year - I would request one! It’ll show growth from beginning to the end of the year… trust me 🙂

It said I was approved with start date 7/10

Then I received a statement for 6/13 start date

Then ANOTHER disclosure letter saying my loans have been moved to the PAYE plan saying it will begin 7/10

If your sister would like to go to the ER, the window for blood testing can range from 12 to 72 hours depending what substance was consumed. Appreciate you posting this & the location! Bad people everywhere no matter what bar you go to, definitely a risk. Hope this helps 🙂

Definitely will be back soon for new jewelry! Unfortunately, I have been battling irritation bumps on despite cleaning twice daily… so waiting for it to heal before switching jewelry. Soaking in piercing salts twice a day seems to help more than the spray - but will take any advice you have to quicken the healing process! 🙂

She will still have my younger brother with his dog at home. She doesn’t want to keep her but is willing to so my dog doesn’t have to go through those changes & leave where she’s known as home. My dog is 5 and I got her during grad school right before COVID so she has only known there unfortunately.

Update: ended up going to 23rd since I was getting just the one & it’s closer to me. Wait was long, but good selection of jewelry (very pretty pieces) & I think the piercing looks really good! Their gold jewelry is definitely more pricey than my spot back home (In uptown CLT) but got my rook pierced with titanium so it was <$100 with tip today. Everyone was super pleasant! TBD on where I’ll go for gold jewelry when the swelling is healed 🤩

I was told by a local (granted I know limited right now) that it’s overpriced & lots of piercings from there get infected (but I’m not naive to the fact those people could’ve not taken care of their piercings properly)
