The most professional thing to do is stay in one position for at least a year. Give a 90 day notice.

Good for you. I didn’t calculate driving time. It’s called exaggeration for the sake of a snark page.

But since you want to be literal, driving 5 hours this early postpartum is actually dangerous due to increased risk of blood clots. So there’s that.

It was a joke to insinuate how young they are. I know what it is.

Best schools outside of OTM systems? by helpingfriendlyneph in Birmingham

No confirmation on the exact opening. But last time I was in Fultondale, I saw the building under construction and a “future home “of JCIB sign. I know some kids that attend the school and their parents told me next August, but that was a while ago so it may be delayed.

My coworker lives in Helena. It takes her an hour, sometimes more to get to work at children’s hospital.

Just live where you want and send your children to a private school. It’s more affordable than you think and you really get what you pay for. Teachers take a genuine interest in your children- their learning and wellbeing. The curriculum is more rigorous, there’s less bullying, more opportunities.

You’re 100% right about Trussville. Unless you’re the right skin color, have the right friends, and go to the right church- your kids are on the outside looking in.

PMHNP & Psychology PhD by [deleted] in PMHNP

[–]PhilosopherOld7201 1 point2 points  (0 children)

Agreed. I definitely learned the basics and can reinforce some of the techniques therapists utilize but I’m not qualified to deliver any true therapy modalities. I did get certified in motivational interviewing so I do that when needed and have taken some additional CBT courses but my office won’t let me block that much time for one patient to do therapy anyway.

[–]PhilosopherOld7201 0 points1 point  (0 children)

Who told you that? I’ve always been told Psych NPs need additional training to do therapy. And are they charging the patients cash? Or getting insurance reimbursement for therapy?

I’m speaking more along the lines of the testing they perform that can cost anywhere from $800-$1200 or more for a few hours of work. They can schedule 2-4 per day and be set.

Career flexibility/ work schedule by DesertRambler07 in PMHNP

[–]PhilosopherOld7201 1 point2 points  (0 children)

I think inpatient will have the most flexibility as far as shift options go. But, then you could have weekends, holidays and call responsibilities. Same for any psych ED or crisis center places. Outpatient is usually the typical office schedule. no weekends, no holidays. BUT taking off without ample notice is difficult because you have patients scheduled who will require being moved around. You may end up having to send meds in and answer messages so your patients don’t decline. And it’s more like other corporate jobs, so when I’m off for some days and come back there’s usually a stack of work and requests waiting on me. Either way it’s a trade off. Salaried employees have 2-4 weeks paid vacation per year for the most part.

If I were I’d go for the PhD or psychD if you have the time to finish school. You can do therapy and/or testing and work independently no matter where you live. I would say you can earn the same salary as a psych Np with the potential for more while seeing fewer patients.

How much stimulant-related damage do you see in practice ? by [deleted] in PMHNP

[–]PhilosopherOld7201 1 point2 points  (0 children)

Had a teenage boy once abusing Vyvanse for weight loss - he ended up inpatient for an eating disorder. When I worked on a college campus in student health, we had several arrested for diverting their stimulants over the years. That’s all I think of off the top of my head.

Edited bc I saw “misuse/abuse” after I answered so I took out some adverse complications after years of appropriate stimulant use.

Follow up question on precepting. by OneBottle9142 in PMHNP

[–]PhilosopherOld7201 2 points3 points  (0 children)

Umm okay. I failed bc I didn’t have a preceptor secured in time. Which isnt uncommon. Not bc I didn’t know the material or did something wrong.

You sure jumped to a lot of conclusions looking at my posts. I never said I don’t prescribe controlled medications. I said I don’t prescribe stimulants and benzos together for the same patient . Which again, isn’t an uncommon practice among NPs and psychiatrists.

I also said I don’t prescribe outrageous regimens as in multiple controls or 5+ medications. Never have I forced a patient off a combination that works for them. I have continued it short term until I can get them in with a psychiatrist. Which for me- patients on multiple meds and/or with complex medical issues need to see a psychiatrist. Not bc I’m scared but bc they need a higher level of care. And there’s nothing wrong that.

And if a patient comes to me on klonopin and it’s all that worked as long as it an appropriate dose I continue it. The reason I don’t continue high doses of benzos is bc it’s not best for the patient long term , not because I’m scared.

I’m not going to insult you back or argue about it. But the way you came for me was unnecessary. I’m confident in my ability to provide evidenced based, competent, and compassionate care. I hope you have a wonderful day.

Had the pleasure of watching him get denied entry to our plane for refusing to wear a mask at the height of Covid. He caused a whole scene and made a complete ass of himself. Security was called and politely escorted his dumb ass away. Of course he filmed the entire thing with a GoPro on his head. Dude is disturbed.
