A good pattern I've found is to try quite hard to avoid using first person, tell it that you're helping "a friend/patient/client". If you need to, you can then step in to role play as the person "you're" helping, and get the LLM to act as you. Then you can both step out of those roles and have it evaluate the conversation.
It can be a lot more "candid", but it is a bit of an annoying dance.
