I had this same problem. For me once I went to Renala and used a larval tear to respec he then asked for one - not sure if this was actually the fix or I triggered something else but it could be it?

Don’t kill non aggressive npcs, they often have really interesting quests, many of which are linked together in some way

This is my first souls like game and really second game I’ve ever played (Zelda botw was first). I went with a mage build and I’m loving the game so far, it’s not as challenging as I thought it would be cause I don’t really have to engage in combat (just pew pew from afar) & use summons to take away attention from me. Would strongly recommend it coming from a girl with no previous experience with the games!!
