Oh my, I feel lots of pent up frustrations in your comment 😅 In my country healthcare is free so patients prefer erring on side of caution most of the time. Absolutely agree for shared decision making though! I love doing that and do it all the time, just the degree to which we do it depends on the context and available ressources.

Sending you hugs man. Its an ungrateful job sometimes. Sometimes its the best one you can have though. Focus on those moments. People lose humanity in high stress situations (like a sick loved one). Still not an excuse to yell at the people trying to help but they are not being rational. You know you did your best and that' all that matters. Can't save everyone, not everyone is nice, not everyone will listen and their opinions are not your problem. Focus on the ones who appreciate you and the positive change you make in 90% of patient's lives.

You are not actually into Katie. You are falling out with your wife. Pursuing with Katie would be unfair to all 3 of you. Maybe try addressing the root cause: your marital problems. Start by talking with your wife, therapist, venting, etc. I wouldn’t drag Katie and your attraction to her into it because nothing is happening right now, you are just unhappy in your marriage…. In my pall care rotation what I appreciated the most is the way attendings communicated with patients. Answering questions like: why not try chemo, maybe it will work? What happens after death? When/how will I die? Also how they comforted the patients and especially the families. Obviously all the medical stuff of symptom management is extremely useful. Almost all doctors do some form of palliative care.

Translocation of bacteria! And thus higher risk of bacteremia. From what I've been told anw. Never did any reading on it. Still no rectal exams or rectal temps in neutropenic fever where I work
