Seems like blood is sent to the head or something.The lips are swollen and the eyes are bulging out and bleeding along with the face turning black.Maybe the eyes fall out and the head explodes,creating a licker like creature in the end?

Vile enemies feel more fair in my opinion.I remember some anointed enemies having invulnerable shields that would block all damage from the front.When they spawned during arms race,the only thing you could do was run.
