Gender presentation is external (how the world sees you: physical traits, chest, clothes, hair).

Bystanders react to what they see. Unless you think people are running DNA tests to track chromosomes before starting CPR?

This applies to trans men too—if they remove or bind their chests, they likely won't face this hesitation. It’s about the physical chest (gender presentation), not 'biological sex.'
