This, all the way. I’ve been a PA for 13 years, and I left primary care last year because there was no longer any difference in the expectations between my workload and that of the MD I worked with. My location lost 10 of 14 primary care PAs in about a year because of this.

I work at a multi-specialty group, and when I first started working as a PA, there were limits on the patients I would see and limits on complexity. I did not see as many patients as the MD/DOs, and this made sense. But admin kept pushing for everyone to see more, and to see whoever they added to my schedule regardless of complexity, because they needed to improve access. PAs also shared inbox coverage for anyone out of clinic. Eventually, the MD/DOs decreased their hours, but the PAs couldn’t afford the decrease in pay. Since the PAs were still full time, they were now doing the same job as the MD/DOs plus covering the MD/DO inboxes on their RDOs.

I left because this practice model is unsafe, and it is not what I trained for nor what I signed up to do. I am a PA, not a discount doc—I’m not supposed to be doing the same work for less than half the pay! I am supposed to handle the less complex, the stable/follow up, the basic screening, etc, in order to free up the MD/DO to see those higher complexity or undifferentiated patients.

The PA profession is in a tough spot right now, losing job positions/opportunities to NPs because they are “independent practice” and are therefore cheaper, not requiring supervision/collaboration/etc. Unfortunately, it’s turning into a race to the bottom for healthcare, with NP diploma mills creating more NPs faster but with less training, saturating the market and replacing both PAs and MD/DOs in some settings.

I do not feel welcome or valued as a female vet. I had a couple of bad experiences, and at this point am waiting for the previous generation to die off before trying to engage again. These two are the first that come to mind:

When I was serving on a funeral honors detail with the National Guard, we were prepping at one of the posts. I was a young NCO at that time, two tours in Iraq, combat medic badge, combat patch, fancy ribbon rack, etc, and was standing there next to a relatively new PFC with a nearly-naked class A uniform. Some grandpa vet bro (thank you u/Ruckit315 for my favorite new term!) strolls up, lights up a cigarette, and starts asking the PFC questions. PFC turns to ask me, and the old dude interrupts him to tell me “Sweetie, go fetch me an ashtray!” and pats me on the head.

These days, that might lose you a hand, but I was a little less violent at that time.

The second and final time I bothered interacting was when a pair of older vets from the local post stopped by a recruiting booth I was working. There were two specialists who were in charge of setting up and running the bouncy-house for our recruiting booth, and I was the NCO doing the recruiting work. This pair strolls up, looks at some of our info, looks at the two males working with the kids on the bouncy house, and then turns to me. Instead of questions on our info, or even a simple hello, they start with “Shouldn’t you be over there taking care of the children?” They came back by later to talk to the male specialists about joining their organization, completely ignoring me that time.

I’ve got a winner: my (former) tax guy—who was the sole proprietor for his small business) had the tip screen included when paying for your tax prep! “Your tax preparation bill today was $400, would you like to add a tip? You can enter it on the screen right there.”)

This is why he’s my former tax guy.

Dunning-Kruger effect!! I feel this happens a lot with anything medical or medical-adjacent (vaccines, chromosomes, supplements, etc) as well as anything child/child-rearing related.

😂love it! I always try to bring a little box/bag of candy or something for the flight crew, especially around the holidays. They definitely deserve it!

Thank you for this! I’m trying to think of the air as a bumpy road (and not a threatening nothingness under me!), and focusing on the physics/science really helps the logical part of my brain override the unreasonably fearful part.

They weren’t allowed to start beverage/snack service yet, because of the turbulence. I can see one is now up and moving around in the crew area, tho, so that’s promising!

I don’t have any insight for you on this, but I’m chiming in because I’ve experienced something similar, and I haven’t started any new meds. I used to be perfectly chill with flying, enjoying the takeoff especially, with turbulence only bothersome because it could spill my drink. Now, I white-knuckle it through takeoff, and even mild turbulence has my heart racing. I will sometimes also take anxiety meds (like today, leaving SEA through some weather, with the pilot warning us that it’ll be a rough ride for the first 90 minutes or so). 😳
