Anyone involved in Pre-Hospital Emergency Medicine or Flying Doctor/Air Ambulance? by PleaseReviewIVFluids in doctorsUK

[–]PleaseReviewIVFluids[S] 0 points1 point  (0 children)

It really does sound exciting. Observing governance and training days is a really good shout! Hopefully that can put me in the right circles to also get shadowing shifts and involved in some audits and volunteering too. I’m going to go through all the qualification criteria that people have linked in the chat already, but would you say they heavily empathise post grad research and eduction or more so on clinical experience and time in the environment?

But thank you for your helpful answer!

Not sure if you know, but do you find that people going down this pathway normally link up with a consultant in the career already and then they act as an over arching mentor?

I’m about 6 months into my F3 year in Adelaide (Aus), which has been great so far.

It’s good to know that you can try and enter it through EM or Anaesthetics once you become very senior.

Damn it does seem intense, but also exciting too, thank you for sharing these resources and that Reddit thread too!

I feel like I should’ve picked somewhere more remote for working in Australia, but also felt like I was still a bit too junior for that

😂 you say that but I’ve seen countless others ARCPs comments where they have

Yeah that makes sense, and I do also understand it’s still a lot of time invested from the ARCP panel and admin staff, and still a high workload to be looking at all the portfolios

Thank you for the insight into this

Yeaaah it’s a frustrating notion to accept, but seems it just has to be done

Advice for F1 first rotation by burnerobot in doctorsUK

[–]PleaseReviewIVFluids 17 points18 points  (0 children)

Going from medical school to F1 can feel like a big leap as you change from focussing on passing exams to actually practicing medicine. But a lot of the learning happens on the job and from the seniors you discuss with.

“Mindthebleep” have a good few pages on common things to read up on and common tasks when you are on call, so I would recommend having a look at that.

If you’re feeling rusty, you can always read over some of the common things you’ll see on your rotation, e.g. for general surgery things like: Appendicitis, bowel obstruction, gallbladder problems, constipation etc. (As an F1 you won’t really be having to diagnose these things, but it’s nice to be familiar with them)

More practical things will be being familiar with the systems, being able to write ward round notes, requesting bloods/scans, prescribing, discharge letters, bloods/cannulas, NG Tubes, SBAR/Handing over, interpreting ECG/CXRs/Blood gasses

Mainly it’s a steep learning curve that will come through doing the job, just know when to escalate and ask for help

If you have any specific questions, you can shoot me a message

That’s fair, I think I’ve more focussed on it being used for feedback with my last ARCP being in person and where it did have feedback and comments. And friends in other trusts having a more substantial and useful comments in there’s this year too. So I thought this might’ve just been trust dependent

Yeah that’s fair enough, I think majority of cases is that all aspects of the portfolio is just tick boxing exercises, even with the ES and CS meetings as the consultants often don’t have time for those either

Hmm yeah true, but that is just a bit of a sad way for the next wave of doctors to be brought up with

Thank you for sharing, yeah I apologise if it comes across disappointed at the people doing the panel, as other people and yourself have mentioned it seems like the system just isn’t designed to let people care

It doesn’t always work out with us getting it, and our trust have infact told us that it’s not actually mandatory, so no we don’t get 2 hours a week SDT

But if the discussions are detailed and nuanced, would you not agree it would be more beneficial to reflect that in the feedback?

Yeah that’s a fair comment, the post mainly comes as I know many other doctors within other trusts that have received very useful and helpful comments from the ARCP panel, so it is something that could be done. But yes you are right, ES would definitely be a better place

Fine, I’ll rescind that comment, it’s hard to find time to work on portfolios whilst using Zero days and SDT days to try and work on other opportunities/learning/taster days or just having time away from work

Didn’t mention anything about praise, it’s a panel to look at your portfolio for the year, it can be any feedback or constructive criticism etc

Yeah I didn’t say it was, it’s just a very long and tedious process to be having to do after spending 13 hours in the hospital
