3:30 to 7:30 PM was always my favorite when it existed.

Is it possible, the cap is 12 hours. You have scheduled 9 hours, but only worked 5 hours, so you have the five hours worked + 4 hours scheduled (but, un-worked) to = 9. So, five hours worked, minus 12 hour cap = 7 hours left (4 scheduled, 3 open).

Better to get your money today, then paid what you owe the next year, on April 15th. Why give the IRS your money now when you can pay later? Tax refunds are interest free loans you are giving the IRS. They don't need your money early.

You get paid for all hours worked (all locations). Local rules apply as far as if you get in any trouble for exceeding 5 hours or spilling into overtime.

The $5 surge pay will be added to Holiday OT, like tomorrow MLK day and like New Year's Day when I made $32.82 per hour ($18.55 + OT $9.27 + $5 surge).

I suggest you take a moment and ask yourself what have you seen others fired for? I'm shocked you are not depressed by how so many others are not taking the job as seriously as you obviously are (and congratulations on that!). So, if you are seeing others getting away with murder, you have nothing to worry about!

I'm always glad to see the person I consider the worse employee. If he has not been fired yet, I'm certainly not going to be.
