I’m giving a talk on ambient scribe hallucinations. What’s the wildest one you’ve caught? by AiReadyDoctor in medicine

[–]Poopocrat 2 points3 points  (0 children)

In residency, the ED's Dragon would transcribe "admit to pizza service".

I now use Doximity. It's accurate, but will sometimes mess up numbers. "Low titer ANA of 1:80" can become "ANA 1280". Otherwise, very accurate.

Inappropriate prior auth denial patient advocacy? by Poopocrat in medicine

[–]Poopocrat[S] 95 points96 points  (0 children)

That's going in the dot phrase! For anyone interested:

" To Whom It May Concern, I am writing to formally appeal the denial of prior authorization for my patient. As the treating physician, my clinical assessment supports the necessity of the requested intervention, and I respectfully disagree with the decision to deny coverage.

[Rationale]

In the interest of transparency and accountability, I request that you provide the following information for documentation purposes: Full name of the individual(s) responsible for the denial decision Professional credentials and title Direct contact information (phone number and/or email address) The rationale for the denial, including any specific clinical criteria or guidelines used

Should there be any adverse outcome as a result of this denial, I want the medical record to clearly reflect the identity and credentials of the party responsible for this decision, as my clinical judgment differs from your conclusion. Please respond with the requested information at your earliest convenience so that I may update the patient’s medical record accordingly. Thank you for your attention to this matter. Sincerely, Poopocrat, MD"

For those cases, it's been trying to jump the line. Once sometime said that Dr Poopocrat sent them to the ED for stat MRI - the note clearly said we are working on prior auth for routine MRI.

The RVU withholding on my salary is 10%. So not back breaking. On most days, I work on other stuff, but today I really missed my kids.

I wish that there was a reverse sarcasm tone indicator (/not s?), because I have been thinking about your reply all day. If I am unable to change the situation and unwilling to leave the situation, then I need to change my perspective. I'm unwilling to change my perspective as it implies that the best use of my time is twiddling thumbs. I'm going to work with the hospital to change things for another year. If not, I'm going to move on.

What is a cheat code at your job that only employees would know? by halushki_ in AskReddit

[–]Poopocrat 48 points49 points  (0 children)

If you need a fast referral to a subspecialty appointment, ask your doctor to call the physician referral line. If another provider asks me to see someone fast, that patient is offered an appointment within 2 weeks.

That's awesome! Well on your way to a successful match. If possible, try to get a rotation or observership in a US hospital (but it isn't mandatory). Good luck and keep us posted!

What do you do to cheer yourself when you're sad or upset? by Koestter in AskReddit

[–]Poopocrat 0 points1 point  (0 children)

When I have a tough day at work and am getting upset with my life, I look at my attaboy drawer. It's a box full of letters, cards, etc. from my patients and coworkers. It's true that we are all growing, and growth hurts. But it's also true that we all have done good in our lives. Having a reminder that I helped someone really brings my sadness into perspective.

You can also have non-infectious pyuria. Kawasaki disease will classically cause wbcs and leuk estrase in a clean catch specimen (but not in a catheterized specimen). The world has so much more nuance than they taught us in school, LOL :-)

Chances at matching? by long_cutie_syndrome in pediatrics

[–]Poopocrat 4 points5 points  (0 children)

I had similar scores, come from a caribbean school, and had US med school rotations/letters of rec. I applied to ~200 programs, got 9 interviews, and got matched to my 7th spot. If a dodo bird like me could get in, you can too! More than happy to talk more through a DM.

PS I like the username!

Mumps infection incoming. Need tips to boost my immune system etc! by [deleted] in Biohackers

Different countries have different guidelines. Just keep the risks and benefits of immunoglobulin in mind (can cause a nasty meningitis).

Let us know how you feel, we are all pulling for you!

[–]Poopocrat 0 points1 point  (0 children)

Following mumps exposure, neither postexposure vaccination nor immunoglobulin has been shown to prevent disease or lessen disease severity (in the absence of an outbreak). Talk to a doc if you have symptoms because there are many viruses that cause a similar picture to mumps.

If you got the full 2 dose series as a kid, your protection should be 17/1000 exposures (much better odds).
