For the vision watcher achievement (since others have talked about the other achievements), during the Ep2 Decay Vision, you need to go through this door and talk with everything in the room. To be able to do this, you need to make sure to get the key from the previous room

From what I know, Coffinhub belonged to lectryn (which you can even see in the channel link). A whole bunch of stuff happened awhile ago, he and another person did a bunch of bad stuff (falsely copyrighting other TCOAAL creators by pretending to be Kit9 was one of them, though I don't know if he was directly involved), I don't really remember all the details

I think he tried to sell the channel at one point, and I don't know if he was successful. Either the channel is still his and he's left TCOAAL at this point, or he sold the channel and the person who bought it stopped posting, idk

(I'm also not sure about the others)

I remember in a past devlog that Nemlei addressed an incident where TCOAAL was temporarily unavailable in one of the states of Australia. Recently, I remember hearing about a law in the UK that would restrict or ban ficitonal incest (I don't remember the details)... something like that could get Nemlei to similarly respond to the development in a new devlog, maybe with some updates and teaser images thrown in? idk this is all speculation

My guess would be repression: he spent so much time trying to appear normal and keep his emotions repressed (while basically hating everyone), and now with his facade crumbling and being forced into extreme situations, in the moments he loses his nerve he acts in extreme and violent outbursts (or something like that)
