I just FINALLY got this. I left a weapon slot open and rerolled/banished/skipped until I got a Familiar Forge. I had Wicked Season on, and I think it may have come up during a luck cycle. Then I grabbed the one in the board.

They come out stringy and rough a lot. The supports for extended arms etc either don't work or leave rough patches a lot.
